摘要
目的观察压疮评估的临床护理路径在结核性脑膜炎患者护理中的应用疗效。方法患者入院时由专业人员采用国际通用的Waterlow scale评分表对其压疮的发病风险进行评估,对不同危险度患者采取不同的护理方法,比较患者住院前后的压疮风险变化和住院前后的Braden Q和Braden评分。结果患者平均住院时间(10.8±2.2)d,住院期间5例(5.56%)患者发生I度压疮,经治疗后好转。压疮发生部位:骶骨2例,左右足跟各1例,右脚趾1例。患者住院前后的Waterlow scale量表评估结果和Braden Q和Braden评分结果差异具有统计学意义(P<0.05)。结论压疮评估临床护理路径的实施可明显降低压疮的发生率,且追踪评分和护理方案的调整有助于增进医患双方的交流沟通,提高患者及家属的满意度。
Objective To investigate the application effect of clinical nursing pathway of pressure sores assessment in the nursing care of patients with tuberculous meningitis. Methods The onset risks of patients' pressure sores on admission were evaluated by professionals using the international Waterlow scale, and then patients with different risk degree were nursed with different nursing measures. The pressure sores risk, Braden Q and Braden scores of patients before and after hospitalization were compared. Results The average hospitalization time of patients with pressure sores was (10.8±2.2) d. There were 5 patients (5.56%) suffering from Grade Ⅰ pressure sores during hospitalization and recovering after treatment. The sites of pressure sores were as follows, 2 eases of around sacrum, 1 case of around heels and 1 ease of around right toe. There were statistical significance between Waterlow scale results and Braden Q and Braden score (P〈0.05). Conclusion The application of clinical nursing pathway of pressure sores assessment can significantly reduce the incidence of pressure sores, and tracking scores and nursing plan adjustment can promote the doctor-patient communication as well as the satisfaction of patients and their relatives.
