皮肤科患者感染病原菌种类及耐药性分析 被引量:7

摘要 目的了解皮肤科感染病原菌的种类及耐药性特点,为医师诊治皮肤感染选择敏感的抗菌药物提供参考。方法 187株病原菌由皮肤科患者感染脓液分离所得,病原菌分离与种型鉴定依据原卫生部临床微生物学检验常规技术操作:产ESBLs菌鉴定选择CLSI 2010年新折点作筛选与确证;MRS采用CLSI头孢西丁纸片法鉴定,药敏采用CLSI指定的K-B法;微生物检验实行全程室内质量控制。结果皮肤科感染病原菌以革兰阳性菌为主,共检出103株占55.1%,革兰阴性菌69株占36.9%,真菌15株占8.0%;金黄色葡萄球菌检出率最高占24.6%,其次是铜绿假单胞菌占16.6%,第3是肠球菌属占12.3%;感染病原菌的耐药性出现上升现象,A群链球菌对抗菌药物敏感性较高,耐甲氧西林葡萄球菌(MRS)分离率为50.0%、产超广谱β-内酰胺酶(ESBLs)细菌检出率达到了42.9%、耐亚胺培南铜绿假单胞菌(IRPA)检出率为16.1%;革兰阳性球菌对糖肽类药物100.0%敏感,革兰阴性杆菌对磺胺类抗菌药物耐药率最高,均>60.0%。结论皮肤科感染病原菌耐药性呈现上升趋势,临床医师应提高病原学检查的比例,依据药敏结果选择高度敏感、性价比合理的药物治疗皮肤科细菌感染。 OBJECTIVE To approach the species and antimicrobial resistance of pathogenic bacteria causing infec-tions in dermatology department so as to provide guidance for treatment of skin infections with sensitive antibiot -ics .METHODS A total of 187 strains of pathogens were isolated from the pus specimens obtained from the derma -tology department patients with infections ,then the isolation and identification of the pathogens were performed according to the conventional procedures of clinical microbiological examination of the original Ministry of Health , the ESBLs-producing strains were identified to screen and confirm by using new breakpoints of CLSI 2010 ,the MRS was identified with the use of CLSI cefoxitin disk method ,and the drug susceptibility testing was performed by means of K-B method designated by CLSI ;the whole process indoor quality control was carried out for the mi-crobial examination .RESULTS The gram-positive bacteria were dominant among the pathogens causing infections in the department of dermatology ,accounting for 55 .1% (103 strains ) ,the gram-negative bacteria accounted for 36 .9% (69 strains) ,and the fungi accounted for 8 .0% (15 strains ) .The detection rate of the Staphylococcus aureus was the highest (24 .6% ) ,followed by the Pseudomonas aeruginosa (16 .6% ) and the Enterococcus spp (12 .3% ) .The drug resistance of the pathogens causing infections showed an upward trend ,the group A Strepto-coccus strains were highly susceptible to antibiotics ; the isolation rate of methicillin-resistant Staphylococcus (MRS) was 50 .0% ,the detection rate of extended-spectrum -lactamases (ESBLs)-producing strains reached to 42 .9% ,and the detection rate of imipenem-resistant P .aeruginosa (IRPA ) was 16 .1% .The drug susceptibility rate of the gram-positive cocci to glycopeptides was 100 .0% ,and the drug resistance rate of the gram-negative ba-cilli to sulfonamides was more than 60 .0% .CONCLUSION The drug resistance of the pathogens causing infections in the de
