摘要
目的应用二维超声斑点追踪技术(2D-STI)观察兔心肌梗死后室壁瘤形成过程中心肌力学演变规律和特点。方法 20只新西兰白兔建立室壁瘤动物模型,4周后,将存活至实验终点且有室壁瘤形成的10只动物纳入研究。使用2D-STI于模型建立术前、术后1、2、3天及1、2、3、4周分别测量左室短轴观心尖水平各节段室壁径向应变率(Sr R)和圆周应变率(Sr C)的收缩期峰值(Sr R-S和Sr C-S)、舒张早期峰值(Sr R-E和Sr C-E)及舒张晚期峰值(Sr R-A和Sr C-A),并测量左室二维常规指标及心功能。结果 1与术前比较,术后各时间段各节段Sr R-S、Sr R-E、Sr R-A、Sr C-S、Sr C-E及Sr C-A均下降(P<0.05)。2术后各时间段比较,术后2天后各节段Sr R-S、Sr R-E、Sr R-A、Sr C-S、Sr C-E及Sr C-A较术后1天下降显著(P<0.05)。3术后2天后各节段间比较,前壁、侧壁Sr R-S、Sr C-S较其他节段下降明显(P<0.05)。4相关性分析显示,前壁、侧壁Sr C-S、Sr R-S与LVEF相关性较好(r分别为0.887、0.891、0.895和0.890,P<0.05)。结论心肌梗死后室壁瘤形成损害了左室各节段心肌应变能力,以收缩期前壁和侧壁下降明显。二维超声斑点追踪技术可作为室壁瘤形成过程中评价心肌力学变化的重要方法。
【Objective】To investigate the changes of myocardial strain in the process of left ventricular aneurysm(LVA) formation after acute myocardial infarction(AMI) in rabbits by two-dimensional speckle tracking imaging technology(2D-STI).【Methods】A total of 20 New Zealand rabbit models of LVA were prepared by ligation of the middle segment of the left anterior descending artery and the left circumflex artery. Four weeks after the procedure,the 10 animals which had formed LVA were enrolled into this study. 2D-STI was performed to measure the changes of radial strain rate(Sr R) and circumferential strain rate(Sr C) at the level of apex in LV short-axis view at the preoperative and postoperative days 1, 2, 3, and weeks 1, 2, 3 and 4.【Results】Compared with the preoperative parameters, there were significant decreases in the Sr R-S, Sr R-E, Sr R-A, Sr C-S, Sr C-E and Sr C-A at the postoperative days 1, 2, 3, and weeks 1, 2, 3 and 4(P〈0.05). Compared with those at the postoperative day 1, there were significant decreases in the Sr R-S, Sr R-E, Sr R-A, Sr C-S, Sr C-E and Sr C-A at the postoperative days 2, 3 and weeks 1,2, 3 and 4(P〈0.05). The anterior wall Sr C-S and Sr R-S, the lateral wall Sr C-S and Sr R-S significantly decreased compared to those of other segments at the postoperative day 2(P〈0.05). The anterior wall Sr C-S, the lateral wall Sr C-S, the anterior wall Sr R-S, and the lateral wall Sr R-S had high correlations with LVEF(r = 0.887, 0.891, 0.895,0.890, respectively, all P〈0.05). 【Conclusions】The Sr R and Sr C at each segment significantly reduce in the process of LVA formation after AMI, especially in the anterior and lateral segments. 2D-STI can be an important method to assess the myocardial mechanics in the process of LVA formation.
