摘要
目的探讨术前应用软尺测量双下肢的长度及术中测量患肢长度结合髋关节松紧度的方法来预防老年股骨颈骨折全髋关节置换(THA)术后下肢不等长的有效性。方法自2010-01—2012-12诊治老年单侧股骨颈骨折124例,62例术前应用软尺测量双下肢长度及术中测量患肢长度结合髋关节松紧度预防THA术后下肢不等长(试验组),62例进行传统THA手术操作(对照组)。结果 124例均获得随访1~12个月。2组切口均愈合良好,术后无髋关节脱位、下肢深静脉血栓形成及深部感染发生。试验组25例下肢延长平均4.0(1~10)mm,下肢延长患者没有出现步态异常。对照组30例下肢延长平均5.3(1~10)mm,没有出现步态异常;15例下肢延长平均12.7(11~20)mm;1例下肢延长30 mm,出现跛行、髋关节疼痛及下肢痛。对照组46例下肢延长平均7.6(1~30)mm。试验组术后肢体延长程度明显较对照组轻,差异有统计学意义(t=2.3,P〈0.05)。结论术前采用软尺测量双下肢长度及术中测量患肢长度结合髋关节松紧度可有效地预防老年股骨颈骨折THA术后下肢不等长。
Objective To investigate the efficacy of application of flexible rule measuring the length of lower limbs before and during the operation in prevention of inequality of lower limb in old patients with femoral neck fracture undergoing total hip arthroplasty. Methods A total of 124 patients with femoral neck fractures were randomly divided into experimental group and control group. All patients were treated with total hip arthroplasty. The experimental group(62 cases) was used to measure the leg length with flexible rule before and during the operation. Control group(62 cases) was treated according to the traditional routine surgical procedure. Results All patients received follow up for 1-12 months. There were 37 cases with the lower limbs isometry, and 25 cases with lengthening of 4.0 mm(1-10 mm) in 62 cases of the experimental group. But in 62 cases of the control group, there were 16 cases with the lower extremities isometry, 30 cases with lengthening of 5.3 mm(1-10mm), 15 cases with lengthening of 12.7 mm(10-20 mm), and 1 case with lengthening 30 mm. Conclusion Application of flexible rule measuring the length of lower limbs before and during the operation can effectively prevent inequality of lower limb in total hip arthroplasty in the treatment of femoral neck fracture
