摘要
目的探究人工股骨头置换术在股骨颈骨折患者中的应用效果.方法选择2021年1月—2022年7月该院收治的120例股骨颈骨折患者为研究对象,采用随机数字表法分为两组,各60例.对照组采用空心螺钉内固定术,观察组采用人工股骨头置换术,两组均观察到术后6个月.对比两组髋关节功能、手术情况、并发症及髋关节恢复优良率.结果术后,观察组关节功能、畸形、疼痛、关节活动度评分及总分分别为(38.64±1.01)分、(3.08±0.36)分、(36.23±3.78)分、(3.89±0.49)分、(82.44±5.04)分,均高于对照组的(34.72±2.89)分、(2.45±0.29)分、(32.43±3.15)分、(3.09±0.38)分、(72.62±4.85)分;观察组髋关节恢复优良率为96.66%,高于对照组的85.00%;观察组手术时间为(93.82±16.78)min、住院时间为(20.59±6.32)d,均短于对照组的(108.04±20.43)min、(30.94±5.56)d;观察组并发症发生率为3.33%,低于对照组的13.33%,组间差异有统计学意义(P<0.05);观察组术中出血量为(329.52±30.59)mL,多于对照组的(320.61±26.92)mL,差异无统计学意义(P>0.05).结论人工股骨头置换术在股骨颈骨折患者中效果显著,能够改善髋关节功能,缩短住院时间,且并发症较少.
Objective To investigate the effect of artificial femoral head replacement in patients with femoral neck fracture.Methods 120 patients with femoral neck fracture admitted to our hospital from January 2021 to July 2022 were divided into two groups with 60 cases each by random number table method.The control group underwent internal fixation with hollow screw,and the observation group underwent artificial femoral head replacement.The results were observed 6 months after operation in both groups.The hip function,operation status,complications and good rate of hip recovery were compared between the two groups.Results After surgery,the scores of the observation group's joint function,deformity,pain,joint range of motion,and total score were(38.64±1.01)points,(3.08±0.36)points,(36.23±3.78)points,(3.89±0.49)points,and(82.44±5.04)points,respectively,which were higher than(34.72±2.89)points,(2.45±0.29)points,(32.43±3.15)points,(3.09±0.38)points,and(72.62±4.85)points in the control group;the excellent and good rate of hip joint recovery in the observation group was 96.66%,which was higher than 85.00%in the control group;the surgical time and hospitalization time of the observation group were(93.82±16.78)min and(20.59±6.32)days,which were shorter than(108.04±20.43)min and(30.94±5.56)days of the control group;the incidence of complications in the observation group was 3.33%,lower than 13.33%in the control group,the difference between groups was statistically significant(P<0.05);the intraoperative bleeding volume in the observation group was(329.52±30.59)mL,which was more than the control group's(320.61±26.92)mL,with no statistically significant difference(P>0.05).Conclusion Artificial femoral head replacement has a significant effect in patients with neck of femur fracture,which can improve hip joint function,shorten hospital stay,and have fewer complications.
