摘要
目的观察外固定支架联合有限内固定治疗胫骨平台骨折的疗效。方法选择2011年1月~2014年1月胫骨平台骨折患者70例,其中男性41例,女性29例;年龄32~66岁,平均年龄41.60岁。根据手术方式不同将患者分为观察组和对照组,每组各35例。观察组予以外固定支架联合有限内固定治疗,对照组予以锁定钢板内固定治疗。观察两组的疗效、手术时间、术中出血量、切口长度、骨痂形成时间、骨折愈合时间、住院时间和并发症发生率。结果观察组优良率为88.57%,对照组优良率为82.86%,两组差异无统计学意义(χ2=0.117,P〉0.05)。观察组手术时间、术中出血量和切口长度均较对照组明显缩短或者减少(P〈0.01),而两组骨痂形成时间、骨折愈合时间和住院时间差异无统计学意义(P〉0.05)。观察组并发症发生率为5.71%,明显低于对照组的25.71%(χ2=3.883,P〈0.05)。结论外固定支架联合有限内固定治疗胫骨平台骨折的疗效确切,具有微创、术后恢复快、并发症少等特点,值得推荐临床应用。
Objective To observe the efficacy of external fixation combined with limited internal fixation in patients with tibial plateau fractures. Methods A total of 70 patients with tibial plateau fractures from January 2011 to January 2014 were enrolled, which included 41 males and 29 females, aged 32 - 66 years old with mean age of 41.60. In accordance with the modus operandi, all of them were divided into observation group(n = 35) and control group(n = 35). The observation group was performed external fixation combined with limited internal fixation and control group was given locking plate fixation. The efficacy, operative time, blood bleeding, incision length, callus formation time, healing time, hospitalization time and complication rate were observed. Results The excellent and good rate was 88.57 % in observation group, and 82.86 % in control group, with no statistically significant difference between 2 groups(X2 = 0.117, P 〉 0.05). The operative time, blood bleeding and incision length in observation group were significantly shorter or lower than those in control group(P 〈 0.01). The callus formation time, healing time and hospitalization time were not significantly different between 2 groups(P 〉 0.05). The complication incidence was 5.71% in observation group significantly lower than 25.71% in control group(x2 = 3.883, P 〈 0.05). Conclusion It is demonstrated that external fixation combined with limited internal fixation in patients with tibial plateau fractures is obviously minimall invasive with rapid postoperative recovery and less complications and other characteristics, and should be recommended for clinical application.
