摘要
目的观察负荷剂量硫酸氢氯吡格雷联合阿司匹林治疗急性进展型脑梗死疗效。方法将64例急性进展型脑梗死患者随机分为治疗组与对照组各32例。对照组采用常规治疗;治疗组在此基础上给予硫酸氢氯吡格雷治疗。经过2周后,比较2组的临床疗效、血小板聚集率、纤维蛋白原、全血黏度(高切)、全血黏度(低切)、C反应蛋白及SNOBS评分。结果治疗组治疗14d后显效率和有效率均高于对照组(P<0.05)。治疗前2组观察指标比较差异均无统计学意义(P>0.05)。治疗14d后2组血小板聚集率、纤维蛋白原水平、全血黏度(高切)、全血黏度(低切)、C反应蛋白水平均低于治疗前,且治疗组低于对照组(P<0.01)。2组治疗前SNOBS评分比较差异无统计学意义(P>0.05)。治疗14d后2组SNOBS评分均高于治疗前,且治疗组高于对照组(P<0.01)。结论进展型脑梗死患者采用负荷剂量硫酸氢氯吡格雷联合阿司匹林治疗,疗效显著,能更有效地降低血脂、凝血因子、炎症因子水平,无明显不良反应,值得临床进一步推广。
Objective To observe clinical effect of load - dose plavix combined with aspirin on acute progressive cer- ebral infarction. Methods 64 patients with acute progressive cerebral infarction were randomly divided into treatment group and control group, each of 32 cases. Control group was given with regular treatment, while treatment group was additionally trea- ted with load-dose plavix combined with aspirin. After 14 days＇ treatment, the curative effects, platelet aggregation, fibrinogen, whole blood viscosity（ high shear）, whole blood viscosity（ low cut）, C-reactive protein and scores of SNOBS were compared be- tween two groups. Results The efficiency and the effective rate of the treatment group were higher than that of control group （ P 〈 0.05）. Before the treatment, the differences of all the observation indexes between the two group were not significant （ P 〉 0.05 ）. After teatment, the platelet aggregation, fibrinogen, whole blood viscosity （ high shear）, whole blood viscosity （ low cut） and C-reactive protein of two group were all lower than that of before treatment, and those indexes of treatment group were lower than that of control group（ P 〈0.01 ）. Before the treatment,the scores of SNOBS had no significant differences between the two groups （ P 〉 0.05 ）. After treatment, SNOBS score of two groups were higher than that before treatment, and that of treatment group than that of control group （ P 〈 0.01 ）. Conclusion Load-dose plavix combined with aspirin has a significant curative effect on acute progressive cerebral infarction and was more efficient in reducing blood lipids, coagulation factors, inflammatory factors. No obvious adverse reactions, and it is worthy of further promotion.
