摘要
目的通过对胎儿静脉导管频谱形态分析,定性评价妊娠高血压疾病对胎儿右心舒张功能的影响,为孕期母婴监测、治疗效果评价提供依据。方法应用彩色多普勒检测92例妊娠高血压孕妇,将其分为重度子痫前期组、轻度子痫前期组和妊娠高血压3组,选择60例与其孕龄匹配、血压正常的孕妇胎儿静脉导管血流频谱,并比较各组胎儿静脉导管血流频谱形态的改变情况,观察频谱中反向A波的发生,通过反向A波的发生定性评价胎儿右心舒张功能。结果正常对照组及妊娠高血压组均未探及反向A波,轻度子痫前期组中有反向A波2例,重度子痫前期组中有反向A波5例,比较轻度及重度子痫前期组患者静脉导管反向A波发生率,重度子痫前期患者静脉导管反向A波发生率高于轻度子痫前期患者,差异有统计学意义(χ2=4.47,P=0.03,P<0.05)。结论通过对胎儿静脉导管频谱形态分析,依据反向A波的发生可定性判断胎儿右心舒张功能的改变情况,为临床提供治疗及预后依据。
OBJECTIVE Through analysis of fetal ductus venosus spectrum morphology qualitatively assess the impact of fight ventricu- lar diastolic function in the fetus with gestational hypertension, and provide the basis for monitoring the mother and child during pregnancy, treatment evaluation. METHODS There were 92 cases of pregnant women with hypertension(divided into three groups of severe preeclampsia, mild preeclampsia, gestational hypertension) were detected by color doppler and there were 60 cases with ges- tational age matched normotensive pregnant women with fetal were deteced ductus venosus blood flow spectrum, compared the change of fetal ductus venosus blood spectrum morphology and observed the reverse A wave spectrum, evaluation of fetal right ven- tricular diastolic function by qualitative reverse A wave. RESULTS The normal control group and gestational hypertension group had no reverse A wave, mild preeclampsia group had two reverse A wave, severe preeclampsia group had five reverse A wave, com- pared the incidence of the mild and severe preeclampsia patients with venous catheter backward wave, the difference was statistical- ly significant(X^2=4.47, P=0.03, P〈0.05). CONCLUSION Through analysis of spectrum morphological with fetal ductus venosus, the reverse A wave could be qualitative judgments fetal right ventricular diastolic function changes, provide the basis for treatment evaluation.
