摘要
目的探讨腹腔镜术联合新辅助放化疗在中低位直肠癌中的治疗效果。方法将我科收治的68例中低位直肠癌患者随机分为观察组和对照组各34例,所有患者均予以新辅助放化疗治疗措施,且对照组给予传统的开腹手术进行治疗,观察组给予腹腔镜手术进行治疗。观察并记录两组患者肿瘤治疗情况(肿瘤切除标本的远、近切断长度以及环周切缘阳性率)、局部复发率、转移率(远处转移率和复发转移率)、3年无瘤生存率及总生存率情况。结果观察组患者淋巴结清扫数目、远切断长度均明显高于对照组,差异具有统计学意义(P<0.05);观察组局部复发率明显低于对照组(P<0.05);观察组转移率,3年无瘤生存率及总生存率与对照组相比差异均无统计学意义(P>0.05)。结论腹腔镜术联合新辅助放化疗可增加淋巴结清扫数目和远切断长度,但是对于无瘤生存率及总生存率影响不明显。
Objective To investigate the therapeutic effect of laparoscopic surgery combined with neoadjuvant chemoradiotherapy in middle and low rectal cancer. Methods 68 patients with middle and low rectal cancer admitted to our department were randomly divided into observation group and control group, with 34 cases in each group. All patients were treated with neoadjuvant chemoradiotherapy, the observation group was treated with laparoscopic surgery, the control group received traditional laparotomy for treating. The tumor treatment conditions（the distal and proximal cut-off lengths of tumor resection specimens, the positive rate of circumferential resection margin）, local recurrence rate, metastasis rate（distant metastasis rate and recurrence metastasis rate）, 3-year disease free survival rate and overall survival rate of the two groups were observed and recorded. Results The number of lymph node dissection and distal cut-off length of the observation group were significantly higher than those of the control group, with statistical difference（P 0.05）; the local recurrence rate of the observation group was significantly lower than that of the control group, with statistical difference（P 0.05）; the metastasis rate, 3-year disease free survival rate and overall survival rate of the two groups had no statistical difference（P 0.05）. Conclusions Laparoscopic surgery combined with neoadjuvant chemoradiotherapy can increase the number of lymph node dissection and distal cut-off length.However, the effect of the method on the disease free survival and overall survival is not significant.
