摘要
目的探讨奥美拉唑、克拉霉素、阿莫西林三联疗法治疗幽门螺杆菌阳性消化性溃疡的疗效。方法选取2013年6月~2014年6月我院收治的消化性溃疡患者80例,随机分为观察组和对照组,每组40例,观察组患者饭前30分钟服用奥美拉唑20 mg、克拉霉素500 mg、阿莫西林1000 mg,早晚各1次,1周为一个疗程,需持续4个疗程。对照组每次予奥美拉唑20 mg,每日2次,疗程4周。结果观察组的总有效率为95%,对照组的总有效率仅75%,两组疗效比较差异具有统计学意义（P〈0.05）。观察组患者的Hp根除率为90%（36/40）,对照组患者治疗后复查有12例Hp未根除,对照组患者的Hp根除率为70%（28/40）。观察组患者的Hp根除率显著高于对照组,差异具有统计学意义（χ^2=6.387,P〈0.05）。观察组患者治疗后的腹痛、腹胀、烧心、反酸、嗳气的临床症状积分均显著低于对照组,差异具有统计学意义（P〈0.05）。观察组患者治疗后的生活质量各项评分均较对照组显著改变,差异具有统计学意义（P〈0.05）。结论奥美拉唑、克拉霉素、阿莫西林三联疗法治疗幽门螺杆菌阳性消化性溃疡疗效确切,可以明显改善临床症状,提高生活质量。
Objective To explore the therapeutic effect of omeprazole,clarithromycin, amoxicillin triple in treatment of Helicobacter pylori positive peptic ulcer. Methods From 2013 June to 2014 June in our hospital, 80 patients with peptic ulcer were randomly divided into observation group and control group, 40 cases in each group, the observation group patients before taking meal 30 min took omeprazole 20 mg,clarithromycin 500 mg, amoxicillin 1000 mg, Morning and evening each time, and 1 weeks as 1 course, continued the 4 courses. Every time,the control group took omeprazole 20 mg, 2 times a day, continued for 4 weeks. Results The total effective rate of observation group was 95%, the total effective rate of control group was 75%, the efficacy were compared between two groups, the difference was statistically significant（P〈0.05）.The Hp eradication rate of observation group was 90%（36/40）, the control group after treatment was 12 cases in Hp was not eradicated, the Hp eradication rate of control group was 70%（28/40）. The observation group of patients with Hp eradication rate was significantly higher than the control group, the difference was statistically significant（χ^2=6.387, P〈0.05）. Clinical symptoms of observation group patients after abdominal pain, abdominal distention, acid regurgitation, heartburn, belching were significantly lower than those in the control group,the difference was statistically significant（P〈0.05）. The score of life quality of the patients after treatment was significantly higher than the control group, the difference was statistically significant（P〈0.05）. Conclusion Curative effect of Omeprazole, clarithromycin, amoxicillin triple in treatment of Helicobacter pylori positive peptic ulcer, can significantly improve the clinical symptoms, improve the quality of life.
