摘要
目的观察补中益气汤对慢性阻塞性肺疾病(COPD)稳定期患者营养状态的影响。方法选取2011年3月—2013年5月来院就诊的90例COPD患者,采用随机数字表法分为对照组和治疗组,每组45例患者,对照组采用传统西医治疗,治疗组在对照组的基础上联合服用补中益气汤;检测对比2组患者的临床疗效、营养学指标、呼吸功能及运动耐力指标。结果与治疗前相比,2组患者中医证候各指标评分治疗后均有明显改善(P<0.05);且治疗后治疗组各项评分的改善情况均优于对照组(P<0.05)。治疗组的总有效率为97.78%,明显高于对照组的77.78%(χ2=8.389,P<0.05)。治疗组患者的IBW%、TSF和MAMC治疗后均明显增加(P<0.05),而对照组则无明显变化(P>0.05);血液检测结果同样发现治疗组的瘦素、TNF-α、IL-18、ALB、PALB、TRF和TLC等指标治疗后均有明显改善(P<0.05),对照组变化差异则无统计学意义(P>0.05)。8周后发现对照组的FEV1(%)与治疗前相比下降明显(P<0.05);治疗组的FEV1(%)和6MCD与治疗前相比均有显著增加(P<0.05)。结论在常规西医治疗的基础上,服用补中益气汤可显著改善COPD稳定期患者的营养状态,提高临床疗效。
Objective To observe the effect of Buzhong Yiqi decoction on the nutrition status of patients with chronic ob- structive pulmonary diseases（COPD）. Methods From March 2011 to May 2013 in our hospital 90 cases of patients with COPD were chosen and randomly divided into two groups（45 cases in each group） ;The control group received the conventional western medicine treatment, while the treatment group patients were given Buzhong yiqi decoction on basis of conventional western medicine treatment;The clinical efficacy, nutrition indexes, respiratory function and sports endurance indexes of both two groups were compared and studied. Results TCM syndrome score indexes in both two groups were all significantly improved after the treatment （ P 〈 O. 05 ） ; And after the treatment, the score improvement of the treatment group were superior to that of the control group （P 〈 0.05 ）. The total effective rate in the treatment group was 97.78% which was obviously higher than 77.78% in the control group（ x2 = 8. 389, P 〈 O. 05 ）. IBW %, TSF and MAMC in the treatment group were significantly increased after the treatment （P 〈 O. 05 ）, while these in the control group had no signifi- cant change（P 〉 0.05 ） ;Blood test results also found that the levels of leptin, TNF-α, IL-18, PALB,TRF and TLC in the treatment were improved significantly after the treatment （P 〈 O. 05 ）, while index changes in the control group had no sig- nificant difference （P 〉 0.05 ）. 8 weeks after the treatment, we found that FEV1 （ % ） in the control group decreased sig- nificantly（P 〈0.05） as compared with that before treatment,while in the treatment group, FEV1 （%） and 6 MCD were all significantly increased as compared with that before treatment （P 〈 O. 05 ）. Conclusion On the basis of conventional western medicine treatment, Buzhong Yiqi decoction could significantly improve the nutritional status of patients with COPD, as well as the clinical curative effect.
