摘要
目的探讨鼻内窥镜联合柯-陆氏双径路手术治疗真菌性上颌窦炎的临床效果。方法将本院收治的69例真菌性上颌窦炎患者随机分为对照组(34例)和观察组(35例)。对照组患者采取单纯鼻内窥镜下手术清除病灶;观察组患者采用鼻内窥镜联合柯-陆氏双径路手术治疗,两组患者术中、术后均以氟康唑注射液彻底冲洗鼻腔和上颌窦窦腔。治疗前及随访时行鼻腔鼻窦结局测试-20(SNOT-20)量表评分,并比较两组患者治疗有效率。结果两组患者治疗有效率比较无显著差异(P>0.05),但观察组患者的SNOT-20评分减少明显多于对照组(P<0.05)。结论鼻内窥镜联合柯-陆氏双径路手术与单纯鼻内窥镜手术治疗真菌性上颌窦炎的有效率相近,但鼻内窥镜联合柯-陆氏双径路手术可以更有效地减轻患者症状。
Objective To investigate the clinical effect of ndoscopic sinus surgery combined with CaldwellLuc surgery in the treatment of fungal sinusitis. Method 69 patients with fungal sinusitis were randomly divided into control group（34 cases） and observation group（35 cases）, control group patients were treated only by endoscopic sinus surgery, observation group were treated by endoscopic and Caldwell-Luc operation for the treatment of dual path, nasal cavity and maxillary sinus cavity were rinse thoroughly by the fluconazole injection and intraoperative. Recorded the SNOT-20 score before treatment and follow-up, and compared the effective rate. Result There was no significant difference between the two groups in effective rate（P〉0.05）. But SNOT-20 score reduction in observation group was higher than control group（P〈0.05）. Conclusion Endoscopic sinus surgery combined with Caldwell-Luc surgery have similar cure rate in the treatment of fungal sinusitis with endoscopic sinus surgery only, but endoscopic sinus surgery combined with Caldwell-Luc surgery can more effectively relieve clinical symptoms.
