摘要
目的比较颈内静脉长期透析导管不同置管方法的难易程度。方法将26例需进行长期颈内静脉置管的维持性血液透析患者随机分为A组(18例)和B组(8例),A组采用传统的撕脱鞘法置管,B组采用导丝法置管,观察两种方法的置管术中出血量、置管所用时间、成功率、置管后透析中的平均血流量。结果 B组在置管所用时间为(43.63±3.74)min,术中出血量为(8.38±2.20)m L,均显著低于A组的(55.72±7.40)min和(24.78±4.69)m L,差异具有统计学意义(P<0.05);而B组的置管成功率为100%,透析中的平均血流量为(242.50±25.91)m L/min,A组的置管成功率为94.44%,透析中平均血流量为(232.78±21.97)m L/min,两者比较差异无统计学意义(P>0.05)。结论采用导丝法进行颈内静脉长期透析导管置入方法简单,且术中出血少,值得临床推广应用。
Objective To compare the complexity of two internal jugular vein indwelling catheter methods in long term of dialysis. Methods 26 cases of maintenance hemodialysis patients needing long term internal jugular vein indwelling catheters were divided into group A （18 cases） and group B （8 cases）. Patients in group A were inserted with catheter by pull-apart sheath method, patients in group B were inserted with catheter by guide wire method. The amount of blood loss during operation, cathetering time, successful rate and the average blood flow during the hemodialysis were observed. Results The cathetering time and the amount of blood loss during operation of group B were （43.63 ± 3.74） min and （8.38 ± 2.20） mL, significantly lower than （55.72 ± 7.40） min and （24.78 ± 4.69） mL of the group A, with statistical significant difference （P〈0.05）; the successful rate of cathetering and the average blood flow during the hemodialysis in group B were 100% and （242.50 ± 25.91） mL/min respectively, in group A were 94.44% and （232.78 ± 21.97） mL/min, the difference of the two groups was not statistical significant （P〉0.05）. Conclusions Guide wire method in long term internal jugular vein dialysis catheters is easy with less blood loss during operation compared with pull-apart sheath method, which deserves promotion and application.
