摘要 目的探讨甲状旁腺激素(PTH)在终末期肾脏病患者血液透析中的意义。方法将100例长期行维持性血液透析的患者分为≤60岁、>60岁组,测定100例患者血液透析前后的PTH、血尿素氮(BUN)、血肌酐(SCr)、收缩压(SBP)、血红蛋白(Hb)、血浆白蛋白(Alb)、血磷(P3+)、血钙(Ca2+)水平等,分析BUN、SCr、SBP、Hb、Alb、P3+、Ca2+等与PTH的相关性。结果透析后,患者的BUN、SCr、SBP、P3+、PTH水平显著低于透析前(P<0.01),而Hb、Alb、Ca2+水平显著高于透析前(P<0.01);透析后,>60岁组患者的PTH水平低于≤60岁组患者(P<0.05);透析前,PTH与BUN、SCr、SBP呈正相关(r=0.148、0.651、0.326),与Hb、Alb、KT/V呈负相关(r=-0.621、0.337、0.598)。结论 PTH能对血液透析充分性进行有效评估。 Objective To discuss the significance of parathyroid hor mone(PTH) in hemodialysis of patients with endstage renal disease. Methods 100 patients with long-term maintenance hemodialysis were divided into the ≤60 years old group and the 60 years old group,and PTH,Blood urea nitrogen(BUN),serum creatinine(SCr),systolic blood pressure(SBP),hemoglobin(Hb),plasma albumin(Alb),blood phosphorus(P^3+),blood calcium(Ca^2+) level of 100 patients before and after hemodialysis were determined.The correlation between BUN,SCr,SBP,Hb,Alb,P^3+,Ca^2+and so on and PTH was analyzed. Results After hemodialysis,BUN,SCr,SBP,P3+,PTH level of patients was obviously lower than that before hemodialysis respectively(P0.01);Hb,Alb,Ca2+of patients was obviously higher than that before hemodialysis respectively(P〈0.01);after hemodialysis,PTH level of the 〉60 years old group was lower than that of the ≤60 years old group(P〈0.05);before hemodialysis,PTH and BUN,SCr,SBP was positively correlated(r=0.148,0.651,0.326),and PTH and Hb,Alb,KT/V was negatively correlated(r=-0.621,0.337,0.598). Conclusion PTH can effectively evaluate the adequacy of hemodialysis.
