多药耐药铜绿假单胞菌的目标性监测 被引量:5

Targeted monitoring for multi-drug resistant Pseudomonas aeruginosa and control strategies

摘要 目的了解多药耐药铜绿假单胞菌(MDRPA)的科室分布及耐药性,探索其医院感染的预防与控制策略。方法对2012年医院分离的多药耐药铜绿假单胞菌采用法国生物梅里埃公司VITEK-2Compact全自动细菌鉴定及药敏分析仪进行细菌鉴定及药敏分析,并进行目标性监测,制定目标监测管理方案,建立危急值管理流程,实施医院感染的预防与控制策略。结果 2012年分离出铜绿假单胞菌共837株,多药耐药铜绿假单胞菌检出84株,检出率10.0%;科室主要分布在ICU、神经内科、老年科、泌尿外科,分别占27.4%、20.2%、15.5%、11.9%;标本主要来源于痰液、尿液及分泌物,分别占65.5%、17.9%、7.1%;84株多药耐药铜绿假单胞菌对临床常用抗菌药物的耐药率均>50.0%;全年未发生医院感染暴发与流行。结论多药耐药铜绿假单胞菌的检出率和耐药率较高,制定并落实目标监测管理方案,及时实施医院感染的预防与控制策略,能有效预防多药耐药铜绿假单胞菌的暴发和流行。 OBJECTIVE To investigate the department distribution and drug resistance of multi-drug resistant Pseudornonas aeruginosa, and to explore prevention and control strategies against multi-drug resistant P. aerugi- nosa. METHODS The bacterial identification and susceptibility analysis of multi-drug resistant P. aeruginosa were carried out by the VITEK2-compact. Targeted surveillance, monitoring and management programs, critical value management processes, prevention and control strategies were carried out for multi-drug resistant P. aeruglnosa isolated in 2012. RESULTS A total of 837 isolates of P. aeruginosa were collected in 2012, and 84 isolates of multi-drug resistant P. aeruginosa were detected, and the detection rate of multi-drug resistant P. aeruginosa was 10. 0%. The multi^drug resistant P. aeruginosa were mainly found in the department of ICU (27. 4%), neurology (20. 2%), geriatrics (15. 5%) and urology wards (11. 9%). Specimens were mainly from sputum (65.5%), urine (17.9%) and secretions (7.1%). The resistance of the 84 isolates of multi-drug resistant P. aeruginosa to the clinical common used antibiotics were all 〉50. 0%. Hospital infection outbreak did not occur throughout the year 2012. CONCLUSION The detection rate and the resistance of multi-drug resistant P. aerugi- nosa were high. Monitoring and management program, the timely implementation of hospital infection prevention and control strategies can be effective in preventing outbreaks of multi-drug resistant P. aeruginosa.
