ICU铜绿假单胞菌医院感染暴发与控制措施 被引量:10

Outbreak and control measures for nosocomial infections caused by Pseudomonas aeruginosain ICU

摘要 目的分析ICU 5例铜绿假单胞菌(PAE)医院感染暴发的原因,为预防与控制医院感染提供依据。方法对ICU 2010年3月23-30日连续发生的5例PAE感染患者,进行临床资料及流行病学调查。结果 ICU入住12例患者中有5例发生PAE肺部感染,感染率为41.7%;5例患者下呼吸道痰标本中PAE药敏谱基本相同;同时,在环境微生物采样中,30份物体表面和手标本中有11例采集到PAE,检出率为36.7%;床单元设置不合理,增加了交叉感染的机会;通过对感染患者进行隔离、规范设置床单元等一系列干预措施后,有效控制了感染。结论 ICU按照医院感染管理规范要求,进行合理布局,设置床单元,隔离患者、加强培训,提高手卫生依从性,对环境、物体表面彻底消毒是控制感染暴发的有效措施。 OBJECTIVE To analyze the causes that lead to the outbreak of nosocomial infections so as to provide basis for the prevention and control of nosocomial infections caused by Pseudomonas aeruginosa in ICU. METHODS We conducted the epidemiological survey and analyzed the clinical data for 5 cases with the infections caused by P. aeruginosa continuously happened from 23 to 30 Mar 2010. RESULTS There were 5 of 12 cases in ICU with lung infections caused by P. aeruginosa with the incidence of 41.7% ; the susceptibility spectrum of P. aeruginosa from lower respiratory tract sputum of 5 cases were basically same, meanwhile, of the environment microbial sampling, 11 of 30 cases of objects surface and hand specimens were detected P. aeruginosa with the detection rate of 36. 7%; the irrational layout of ICU increased the chance of cross-infections; in order to effectively control the P. aeruginosa infections, we took a series of intervention measures including the isolation of the patients with infection and the standardized redistribution of bed unit. CONCLUSION According to the hospital infection control standards, we make the layout of ICU be rational, set up unit bed, isolate the patients, intensify the training, and enhance the compliance of hand hygiene. The most effective measure of controlling the outbreak of infections is the thorough disinfection of the ohject surface.
