摘要
目的:探讨抗生素联合血必净治疗细菌性肝脓肿的疗效。方法:将132例细菌性肝脓肿患者,随机分为观察组(66例)和对照组(66例)。对照组在行经皮肝穿刺腔内抽脓术后给予抗生素抗感染治疗,观察组在对照组基础上给予血必净注射液治疗。比较两组治疗前后血常规、肝功能和细胞因子水平的变化及临床症状的消失时间。结果:观察组治疗后白细胞计数和肝功能指标谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)均明显低于治疗前和对照组水平,差异具有统计学意义(P<0.05);观察组体温、肝功能、血常规恢复正常及临床症状消失的平均时间均明显短于对照组,差异具有统计学意义(P<0.05);观察组白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平低于治疗前和对照组水平,差异具有统计学意义(P<0.05);观察组CD4+和CD8+水平及CD4+/CD8+比值较治疗前提高,且明显高于对照组治疗后水平,差异具有统计学意义(P<0.05)。结论:抗生素联合血必净注射液能显著改善细菌性肝脓肿患者血常规、肝功能,缩短临床症状消失时间,该疗效与其抗感染能力以及抑制炎性介质的释放功能有关。
Objective： To investigate the efficacy of antibiotics combined with Xuebijing injection for the patients with bacterial liver abscess. Methods： 132 patients with bacterial liver abscess were randomly divided into the observation groups （66 cases） and the control groups （66 cases）. The control group was given antibiotics after underwent percutaneous transluminal aspiration treatment while the observation group was given Xuebijing injection based on the control groups. The liver function, blood routine, cytokine levels and the disappearance time of the clinical symptoms in the patients of two groups before and after treatment were determined and compared. Results： The white blood cell count and liver function indexes of ALT, AST and TBIL in the observations after treatment group were significantly lower than that of before treatment and the control groups （P〈0.05）. The average time of the temperature, liver function and blood routine return to normal and clinical symp- toms disappearance in the observation groups were significantly shorter than that in the control groups （P〈0.05）. The level of IL-6, TNF-α in the observation groups were significantly lower than that of before treatment and the control group （P〈0.05）. The levels of CD4＋ and CD8＋ levels and CD4＋/CD8＋ in observation groups were significantly higher than that of the control groups （P〈0.05）. Conclusions.. Antibiotics combined with Xuebijing injection could significantly improve the blood routine, liver function and shorten the disappearing time of clinical symptoms of bacterial liver abscess patients. The curative effect and the ability of anti infection and on the release function of inhibiting inflammatory mediators.
