摘要
目的研究正常Lisfranc关节形态学参数。方法收集2013年1月至2014年6月66例(男42例,女24例)正常Lisfranc关节16排螺旋CT扫描薄层原始数据。采用表面重建法重建Lisfranc关节三维图像,应用三维拓扑窄区分割技术提取Lisfranc关节骨性结构,应用三维空间点与线二元素结合的测量模式进行Lisfranc关节相关参数测量。结果榫孔内缘深度为(10.67±2.18)mm(6.60-12.90 mm),榫孔外缘深度为(3.77±1.49)mm(1.60-6.20 mm),榫孔内、外缘深度比值为3.41±1.74(1.21-7.06),第2、3跖骨基底部背侧缘成角25.82°±6.09°(16.00°-35.00°),第4、5跖骨基底部背侧缘成角16.36°±8.26°(6.00°-35.00°),中间柱与外侧柱跖骨基底部背侧缘成角22.09°±4.89°(14.00°-32.00°)。男性组与女性组相比榫孔内缘深度(t=11.442,P=0.000),第4、5跖骨基底部背侧缘成角(t=3.282,P=0.002)及中间柱与外侧柱跖骨基底部背侧缘成角(t=3.134,P=0.003)差异具有统计学意义。结论 Lisfranc关节骨性结构在冠状位呈拱形桥状排列,有利于其稳定。第2跖骨基底部插入内、外侧楔骨组成的榫卯样结构中,榫孔内、外侧缘深度是维持Lisfranc关节稳定性的重要因素。
Objective To research the anatomical data of normal Lisfranc joint based on three-dimensional CT reconstruction.Methods The data of 66 normal feet from January 2013 to June 2014 was analyzed.We measured the medial and lateral depth of the mortise,the angle between the dorsal line of the second and third metatarsal base,the angle between the dorsal line of the fourth and fifth metatarsal base,the angle between the dorsal line of the intermedial and lateral column.Results The inside diameter of the mortise was 10.67±2.18 mm,the outer diameter of the mortise was 3.77±1.49 mm.The angle between the dorsal line of the second and third metatarsal base was25.82°±6.09°,the angle between the dorsal line of the fourth and fifth metatarsal base was 16.36°±8.26°,the angle between the dorsal line of the intermedial and lateral column was 22.09°±4.89°.Conclusion The way that the five metatarsal bases arrange makes a contribution to the stability of the Lisfranc joint.The medial and lateral depth of the mortise influences the stability of the Lisfranc joint significantly.
