摘要
目的 探讨病灶显微切除联合透明质酸钠点眼在真菌性角膜溃疡治疗中的效果.方法 临床病例对照研究.对2006年至1012年12月在解放军第180医院眼科中心回顾性分析接受治疗的真菌性角膜溃疡患者211例211只眼.其中130例130只眼作为治疗组,在接受传统抗真菌治疗基础上,予病灶显微切除清除溃疡表面坏死组织,并予玻璃酸钠眼液点眼促进溃疡愈合;另外81例81只眼作为对照组,仅接受与治疗组相同的传统抗真菌治疗.对比分析两组的住院时间、治愈好转率及最终视力.结果 治疗组平均住院时间（12.15±5.23）d,治愈122例,治愈率93.85％ （122/130）,好转5例,占3.85％,无效3例,占2.31％,最终视力＞0.3者79例,占60.77％ （97/130）.对照组平均住院时间（17.26±6.23）d,治愈69例,治愈率85.19％ （69/81）,好转9例,占11.11％,无效3例,占3.70％,最终视力＞0.3者32例,占39.51％ （32/81）.经统计学分析,治疗组的住院时间短于对照组,治愈率、最终视力均高于对照组,差异具有统计学意义（P＜0.05）,两组的有效率差异无统计学意义（P＞0.05）.结论 病灶显微切除联合玻璃酸钠点眼可促进真菌性角膜溃疡愈合,缩短治疗时间,减少角膜瘢痕,提高治愈率和最终视力.
Objective To analyze the effect of repeated microscopic lesion excision combined with sodium hyaluronate eye drop in the treatment of fungal corneal ulcer.Methods A retrospective analysis of 211 cases （211 eyes） of fungal corneal ulcer in our hospital,130 cases （130 eyes） selected as the treatment group received repeated microscopic lesion excision to remove necrotic tissue of ulcer surface,and sodium hyaluronate eye drops to promote ulcer healing,based on conventional antifungal therapy; 81 cases （81 eyes） as a control group received only the conventional antifungal treatment.The average hospital stay,the cure rate,effective rate and final visual acuity of two groups were comparative analyzed.Results On 130 cases of the treatment group,the average hospital stay was 12.15±5.23d,122 cases were cured,the cure rate 93.85％,5 cases improved,accounted for 3.85％,3 cases were ineffective,accounted for 2.31％,79 cases＇ final visual acuity ＞ 0.3,accounted for 60.77％.On 81 cases of the control group,average hospital stay was 17.26 ± 6.23d,69 cases were cured,the cure rate 85.19％,improved in 9 cases,accounted for 11.11％,3 cases ineffective,accounted for 3.70％,32 cases＇ final visual acuity ＞0.3,accounted for 39.51％.After statistical analysis,the average hospital stay of the treatment group was shorter than that of the control group,the cure rate and final visual acuity were higher,the difference was statistically significant （P ＜0.05）,the two groups had no significant difference in effective rate （P ＞0.05）.Conclusions Repeated microsurgical resection of lesions combined with sodium hyaluronate eye drop can promote fungal corneal ulcer healing,shorten treatment time and reduce corneal scarring,improve the cure rate and final visual acuity.
