国产替比培南体外抗菌活性 被引量:1

In vitro antibacterial activity of domestic tebipenem

摘要 目的评价国产替比培南对我国2009--2011年临床分离致病菌的体外抗菌作用及影响因素。方法2009-2011年全国17个城市19家医院临床分离致病菌557株,包括革兰阳性菌213株,革兰阴性菌307株,厌氧菌37株。采用标准琼脂二倍稀释法测定替比培南及对照药的最低抑菌浓度(MIC),并对其中52株菌进行了不同细菌接种量(1×10^5、1×10^6、1×10^7、1^10^8CFU·mL^-1)和培养基不同pH值(pH5.0、pH6.0、pH7.0、pH8.0、pH9.0)、不同马血清含量(0%、25%、50%、75%)条件下的替比培南MIC测定。结果替比培南对甲氧西林敏感葡萄球菌、链球菌、肠杆菌科细菌、嗜血杆菌、卡他莫拉菌以及厌氧菌均具有良好抗菌作用,其体外抗菌活性优于亚胺培南2~8倍,优于法罗培南4—16倍。特别对于青霉素敏感肺炎链球菌和化脓性链球菌,替比培南的MIC值低于0.002mg·L^-1,显示了很强的抗菌作用。各影响因素测定结果表明,细菌接种量、培养基pH值6.0~9.0以及马血清含量在25%以内对替比培南MIC值基本无影响。当培养基pH值为5.0时,可使金黄色葡萄球菌的MIC值明显下降;培养基中加入50%或更多马血清,可使替比培南对所测多数菌的MIC上升2~16倍。结论国产替比培南对我国2009-2010年临床分离革兰阳性及阴性致病菌均具有很强的抗菌作用,值得进一步研究。 AIM To evaluate the in vitro antibacterial activity and influence factor of domestic tebipenem against clinical isolated bacteria in China from 2009 to 2011. METHODS Five hundred and fifty-seven strains (including 213 gram-pasitive, 307 gram-negative and 37 anaerobe) isolated from 19 hospitals in 17 cities in China from 2009-2011 were collected for minimal inhibitory concentration (MIC) test of tebipenem and control drugs using the twofold agar dilution method. Fifty- two strains were selected to test MIC of tebipenem under various culture conditions such as inoculum size (1×10^5、1×10^6、1×10^7、1^10^8CFU·mL^-1), pH of medium (pH5.0, pH6.0, pH7.0, pH8.0, pH9.0) and horse serum levels (0%, 25%, 50%, 75%). RESULTS Tebipenem exhibited excellent antibacterial activity against methicillin-susceptibility Staphylococcus spp., Streptococcus spp., Enterobacteriaceae, Haemophilus spp., MoraxeUa catarrhalis and anaerobes. Antibacterial activities of tebipenem were 2 - 8 times better than imipenem, 4 - 16 times better than faropenem. Especial for penicillin-susceptibility Streptococcus pneumoniae and Streptococcus pyogenes, tebipenem showed great antibacterial activity with MIC values lower than 0.002 mg·L-1. There were no significant effects of inoculum size, pH 6.0 - 9.0 and less 25% horse serum on MICs. The MIC values of tebipenem against Staphylococcus aureus were decreased when pH was 5.0. The MIC values increased 2 - 16 times when horse serum was increased to 50% or more. CONCLUSION Domestic tebipenem showed excellent in vitro antibacterial activity against clinical isolated gram- positive and gram- negative bacteria in China from 2009--2011 and deserved further study.
