摘要
目的:研究糖化血红蛋白(HbAlc)、空腹血糖(FPG)浓度变化对急性心肌梗死(AMI)合并2型糖尿病(2DM)患者预后的临床价值。方法:选择2011年1月至2014年3月发病时间〈12h的心肌梗死(STEMI)195例患者,根据入院时HbAlc水平分为2组,即75例HbAlc升高组(HbAlc≥6.5%)与120例对照组(HbAlc〈6.5%),分析两组患者的HbAlc.FPG浓度与急性心肌梗死合并糖尿病患者预后的关系。结果:bAlc升高组患者住院期间发生心力衰竭、心源性休克及病死率明显高于对照组,差异有统计学意义(P〈0.01);两组患者住院期间发生心室颤动及室性心动过速差异无统计学意义(P〉0.05)。结论:HbAlc水平、FPG浓度升高是住院期间发生心力衰竭及病亡率的强预测因子,因此,应该重视HbAlc的水平,充分估计病情,并指导患者加强血糖控制。以杜绝、减少患者住院期间并发症的发生,进一步改善AMI患者合并2DM患者的预后,具有十分重要的临床价值。
Objective : To research the clinical significance of glycated hemoglobin and fasting blood glucose concentrations on prognosis of acute myocardial infarction in patients with diabetes meUitus. Methods : 195 cases of onset 〈 12h of AMI with diabetes were divided into two groups according to the acmission HbAlc levels from January 2011 to March 2014,namely 75 cases HbAlc elevated group (HbAlc ≥ 6.5% ) and 120 cases HbAlc control group (HbAlc 〈 6.5% ).Analyze the relationship between the groups of patients with HbAlc,FPG concentrations and prognosis of acute myocardial infarction in patients with diabetes mellitus.Results : HbAlc elevated group occurred during hospitalization in patients with heart failure,cardiogenic shock mortality was significantly higher compared with control group(P 〈 0.01).There were no statistically significant difference in ventricular fibrillation and ventricular tachyeardia occurred during the two groups of patients hospitalized(P 〉 0.05). Conclusions : HbAlc levels and FPG concentration is a strong predictor of heart disease and death rates during hospitalization of AMI in patients with diabetes mellitus.Therefore, we should pay attention HbAlc levels and FPG concentration,fully estimate the disease,and to guide the patients to strengthen glycemic control in order to prevent and reduce complications during hospitalization, further improve the prognosis of AMI in patients with diabetes.These have important clinical value.
