QTc对慢性心力衰竭患者左心室舒张功能不全的预测价值研究 被引量：3

摘要 目的:分析慢性心力衰竭(简称心衰)患者QTc间期与左心室舒张功能的关系,评价QTc对左室舒张功能不全的预测价值。方法:选取138例窦性节律的慢性心衰患者,根据二尖瓣环早期舒张速度(E′)分为舒张功能正常组(≥8ms/s)和异常组(<8ms/s),比较两组患者的差异;运用直线相关分析QTc与左室舒张功能的关系;用ROC曲线计算QTc对左室舒张功能不全的预测特征。结果:与正常组相比,异常组心衰年龄较大(64.5±6.5岁vs 60.8±6.7岁,P<0.001),QT(412.4±32.2ms vs 389.1±21.7ms,P<0.001)和QTc(444.8±26.0ms vs 420.7±20.1ms,P<0.001)较长;QTc与E′负相关(r=-0.55,P<0.01);ROC曲线下面积为0.78(95%CI:74-0.85,P<0.05),QTc临界值为438ms,预测舒张功能不全的敏感度和特异度分别为0.85、0.79;QTc间期长于438ms患者的左室舒张功能指标均较差。结论:在慢性心衰患者中,QTc间期与左室舒张功能有较好的相关性,对左室舒张功能不全有良好的预测价值。
