摘要
目的 探讨吡格列酮联合克罗米芬治疗多囊卵巢综合征的疗效及对身体质量指数( BMI )和内分泌激素的影响。方法 将锦州市中心医院于2011年1月~2013年6月收治并诊断为多囊卵巢综合征的72例患者按就诊顺序随机分为研究组和对照组各36例,研究组患者使用吡格列酮联合克罗米芬治疗,对照组患者单用吡格列酮治疗,比较两组患者治疗有效率,及治疗前后BMI和内分泌激素水平的变化。结果 研究组患者治疗有效率为94.44%,明显高于对照组75.00%(χ^2=5.258, P=0.022)。治疗后两组患者BMI、卵泡刺激素、泌乳素、空腹胰岛素较治疗前均有所下降,但差异无统计学意义( t值分别为0.390、0.271、0.336、1.167,均P>0.05)。研究组治疗后血清血清黄体生成素和雌二醇水平较对照组显著升高( t值分别为2.439、2.141,均P<0.05),而血清雄激素(T)较对照组显著降低(t=2.781,P<0.05)。结论 吡格列酮联合克罗米芬较单用吡格列酮治疗多囊卵巢综合征患者临床疗效更显著,可明显减轻患者体重,降低患者血糖水平及改善内分泌激素紊乱,临床上值得推广应用。
Objective To explore the therapeutic effect of Pioglitazone combined with Clomiphene in treatment of polycystic ovarian syndrome ( PCOS) and its influence on BMI and endocrine hormone .Methods Totally 72 cases of patients with PCOS admitted in Jinzhou Central Hospital from January 2011 to June 2013 were randomly divided into two groups according to visiting order .The study group was treated with Pioglitazone combined with Clomiphene , while the control group was treated with Pioglitazone .The treatment efficiency and the changes of BMI and endocrine hormone levels were compared between two groups .Results The effective rate of the study group (94.44%) was significantly higher than that of the control group (75.00%) (χ^2=5.258,P=0.022).The BMI, FSH, PRL and INS levels after treatment in two groups were lower than those before treatment , but there were no significant differences (t value was 0.390, 0.271, 0.336 and 1.167, respectively, all P 〉0.05).After treatment serum levels of LH and E2 of the study group were significantly increased compared with those of the control group (t value was 2.439 and 2.141, respectively, both P〈0.05), but serum T level decreased significantly (t=2.781, P〈0.05).Conclusion Pioglitazone combined with Clomiphene has better curative efficacy on the treatment of PCOS than single Pioglitazone , which can significantly reduce weight , reduce blood glucose level and improve endocrine hormone disorder . So it is worthy of clinical application .
