摘要
目的探讨泼尼松联合吗替麦考酚酯治疗对Ig A肾病伴新月体形成患者的临床疗效及对脂联素水平的影响。方法将84例患者随机分为观察组及对照组,各42例。对照组给予泼尼松联合米氟特片治疗,持续治疗8周。观察组给予泼尼松联合吗替麦考酚酯治疗。观察两组治疗效果及不良反应,对比患者治疗前后血压、24 h尿量、尿素氮(BUN)、血肌酐(血Scr)、24 h尿蛋白(24 h Uab)、内生肌酐清除率(Ccr)和脂联素水平的变化。结果观察组总有效率为95.24%,明显高于对照组的76.19%(P<0.05),而不良反应率为4.76%,明显低于对照组的21.43%(P<0.05)。观察组治疗后收缩压(SBP)、舒张压(DBP)均低于对照组,而24 h尿量大于对照组(P<0.05)。观察组治疗后BUN、血Scr及24 h Uab低于对照组,而Ccr大于对照组(P<0.05)。观察组脂联素水平下降幅度大于对照组(P<0.05)。结论泼尼松联合吗替麦考酚酯可提高Ig A肾病伴新月体形成患者的临床疗效,改善患者肾功能和代谢状况,安全有效。
Objective To investigate the clinical efficacy of prednisone combined mycophenolate mofetil（MMF） in treating Ig A nephropathy complicating crescent formation and the influence on adiponectin. Methods 84 patients with Ig A nephropathy complicating crescent formation were randomly divided into the observation group and the control group,42 cases each group. The control group was given prednisone combined with Leflunomide Tablets for consecutive 8 weeks,while the observation group was given prednisone combined with MMF for 8 weeks. The curative effects and adverse reactions were observed. The changes of blood pressure,24 h urine output,blood urea nitrogen（BUN）,serum creatinine（Scr）,24 h urine protein（24 h Uab）,endogenous creatinine clearance rate（Ccr） before and after treatment were compared between the two groups. Results The total effective rate in the observation group was 95. 24%,which was significantly higher than 76. 19% in the control group（P〈0. 05）,the occurrence rate of adverse reactions in the observation group was 4. 76%,which was significantly lower than 21. 43% in the control group（P〈0. 05）. The levels of systolic blood pressure（SBP）and diastolic blood pressure（DBP） in the observation group were lower than those in the control group,while 24 h urine output was greater than that in the control group. BUN,serum Scr and 24 h Uab in the observation group were lower than those in the control group,while Ccr was greater than that in the control group（P〈0. 05）. The decrease amplitude of adiponectin in the observation group was greater than that in the control group. Conclusion Prednisone combined with mycophenolate mofetil can increase the clinical effect in the patients with Ig A nephropathy complicating crescent formation,improves the patients＇renal function and metabolism status and has safety and effectiveness.
