多参数MRI和超声造影指导下穿刺活检诊断前列腺癌效果观察 被引量:10

摘要 目的观察多参数MRI和超声造影(CEUS)指导下经直肠前列腺穿刺活检诊断前列腺癌的效果。方法30例疑似前列腺癌患者穿刺前先行MRI检查。穿刺时先行经直肠超声(TURS)检查大致定位可疑位点所在前列腺平面和位置,行TURS指导下常规四区十二针法穿刺活检。固定前列腺平面,注射造影剂,行CEUS检查观察可疑位点血流变化情况,结合MRI检查结果明确定位可疑位点,然后对可疑位点行经直肠前列腺靶向穿刺活检。结果手术切除病灶病理检查结果共检出前列腺癌15例,以其为金标准,则TRUS指导下前列腺穿刺活检检出10例,检出率33.33%,灵敏度66.67%,特异度60%;多参数MRI+CEUS指导下前列腺靶向穿刺活检检出14例,检出率46.67%,灵敏度93.33%,特异度86.67%。多参数MRI+CEUS指导下前列腺靶向穿刺活检诊断前列腺癌检出率、灵敏度和特异度均明显高于TRUS指导下前列腺穿刺活检,P均<0.05。结论多参数MRI和CEUS指导下前列腺靶点穿刺活检诊断前列腺癌效果优于单纯TRUS指导下前列腺穿刺活检。
