摘要
目的:研究hTERT和PTEN基因在肝细胞肝癌中的表达情况,探讨其在肝细胞肝癌发生发展中的作用及意义。方法:应用免疫组化(SP法)和半定量逆转录聚合酶链反应(RT-PCR),同步检测78例肝细胞肝癌及对应癌旁组织和12例正常肝组织中hTERT蛋白和mRNA、PTEN蛋白和PTEN mRNA表达情况。分析hTERT和PTEN基因表达及肝细胞肝癌临床病理参数的关系。结果:hTERT蛋白主要定位于肝癌细胞胞质内,偶见核内染色。hTERT蛋白和mRNA在肝细胞肝癌中的阳性率分别为84.62%(66/78)和78.21%(61/78),均高于癌旁肝组织的阳性率10.26%(8/78)和8.97%(7/78),差异有统计学意义,P<0.01。hTERT蛋白及其mRNA在12例正常肝组织均未见表达。PTEN蛋白明确定位于肝细胞胞质内。PTENmRNA和蛋白在肝细胞肝癌中的阳性率分别为41.03%(32/78)和43.59%(34/78),明显低于癌旁肝组织的阳性率100.00%(78/78),差异有统计学意义,P<0.01。统计学分析显示,在肝细胞中hTERT基因的表达与肿瘤分期分级、门静脉有无癌栓、血清中甲胎蛋白表达水平、是否合并HBV感染等因素有关,P<0.05;与患者年龄、性别、肿瘤大小等因素无关。相关分析显示,PTEN与hTERT在肝细胞肝癌中的表达呈负相关,P<0.05。结论:hTERT和PTEN表达呈负相关,提示PTEN可能在肝细胞肝癌的发生发展过程中对hTERT的活化起着调控作用,其表达有望成为肝细胞肝癌的重要分子生物学指标。
OBJECTIVE:To investigate the expressions of hTERT gene and PTEN gene in human hepatocellular car- cinoma (HCC) tissues and discuss its significance during hepatocellular carcinogenesis. METHODS: Expressions of hTERT and PTEN protein in HCC and the paired para-carcinoma liver itssues from 78 HCC patients and in 12 normal, liv- er tissues were detected by immunohistochemistry (SP method), and hTERT and PTEN mRNA expressions were ana- lyzed by reverse transcription polymerase chain reaction(RT-PCR). The relation between expressions of hTERT,PTEN gene and clinicopathologic parameters in patients with HCC was analysed. RESULTS: Expression of hTERT protein was mainly located in the liver cancer cell plasma, occasionally in cell nucleus. The positive rates of hTERT protein and hTERT mRNA in HCC were respectively 84. 62% (66/78) and 78. 21% (61/78) which were prominently higher than those in the paired para-carcinoma liver tissues which were 10.26 % (8/78) and 8.97 % (7/78) ,P〈0.01. The expression of PTEN protein was definitely located in cytoplasma of HCC. The positive rate of PTEN mRNA and PTEN protein in HCC was respectively 41.03 % (32/78) and 43. 59 % (34/78), significantly lower than that in the corresponding para-car- cinoma liver tissues 100.00% (78/78) ,P〈0.01. hTERT protein and hTERT mRNA were not found in normal liver tis- sues. Chi-square test and t-test analysis revealed that hTERT gene expression was carrelated with tumor clinical grade and stage,portal cancer embolus, HbsAg(+) and high AFP level (P〈0.05). There was a negative correlation between PTEN and hTERT gene expression in HCC (P〈0.05). CONCLUSIONS: hTERT gene expression may play an importantrole in the genesis and development of HCC. HTERT gene may serve as an important molecular-biological indicator in di agnosing and predicating the biological behavior of HCC.
