摘要
目的研究新生血管在胃病发展不同时期胃黏膜中的生成情况及血管内皮生长因子(VEGF)-A/血管内皮生长因子受体(VEGFR)-2的调控作用。方法选择2012年1-12月上海长征医院胃镜室收集非癌变胃黏膜组织(包括慢性胃炎、胃息肉、胃溃疡、肠腺化生)及病理科收集胃癌组织(包括癌组织、癌旁近端、癌旁远端)各20例。采用免疫组化SP法检测微血管密度(MVD);Western Blot及RT-PCR法检测VEGF-A/VEGFR-2的表达。结果新生血管在慢性胃炎、胃息肉、胃溃疡、胃黏膜肠腺化生、胃癌不同区域的表达存在差异,其中,在胃炎胃黏膜中新生血管相对稀疏,而胃息肉、胃溃疡和胃黏膜肠腺化生新生血管稍密集,胃癌新生血管明显密集,有形成血管腔形状。总体上,MVD随着组织恶性转化倾向存在递增趋势,胃炎低于胃息肉、胃溃疡、胃黏膜肠腺化生、癌旁远端,低于癌旁近端,低于胃癌(P〈0.05)。VEGF-A/VEGFR-2对新生血管存在明显的调控作用,其表达和MVD变化高度一致。结论微血管生成贯穿在胃黏膜病变不同时期,VEGF-A/VEGFR-2信号通路对新生血管具有调控作用,抑制微血管生成可能具有阻断胃黏膜恶性病变的意义。
Objective To investigate the expressions of angiogenes and regulation of VEGF-A/VEGFR-2 in gastric mucosa in different development periods. Methods The non-cancerous of gastric mucosa(including 20 cases of chronic gastritis, 20 cases of gastric polyps, 20 cases of gastric ulcer, 20 cases of intestinal metaplasia) were collected in Gastroscope Room of Shanghai Changzheng Hospital, and gastric cancer tissue(including 20 cases of cancer tissues, 20 cases of adjacent proximal, 20 cases of adjacent distal) were collected in Department of Pathology from January to December 2012. The immunohistochemical SP method was used to detect microvessel density(MVD). Western blot and RT-PCR were used to detect the expression of VEGF-A/VEGFR-2. Results There were many differences between the expressions of angiogenes in chronic gastritis, gastric polyps, gastric ulcer, gastric intestinal metaplasia and different regions of gastric carcinoma. The microvascular in patients with gastritis was relatively sparse; gastric polyps, gastric ulcer, gastric intestinal metaplasia were slightly more dense; gastric cancer was more intensive. The microvascular in patients with gastric cancer had vascular cavity shape. In general, the MVD had a trend of increasing along with the changes of tissue malignant degree. MVD in patients with gastritis were lower than gastric polyps, gastric ulcer and gastric intestinal metaplasia, adjacent distal, lower than adjacent proximal, lower than gastric cancer(P〈0.05). VEGF-A/VEGFR-2 had an obvious regulation effect for angiogenesis, the expression and MVD changes was highly consistent.Conclusion Angiogenesis consists in different periods of gastric mucosal lesions. VEGF-A/VEGFR-2 signal pathway has a role in the regulation of angiogenesis. Maybe, there is a meaning that to inhibite angiogenesis can block the gastric mucosal lesions.
