摘要
目的了解成都市主城区狂犬病免疫预防门诊(犬伤门诊)业务管理现状,为进一步规范管理提供依据。方法采用定量和定性调查相结合的方法,在成都市主城区21个犬伤门诊对负责人开展问卷调查统计得分,对医务人员进行定性访谈调查。结果定量调查市及以上、区和社区3个级别犬伤门诊的冷链管理制度(H=0.75,P>0.05)、知情同意书制度(H=1.73,P>0.05)、不良反应登记报告制度(H=3.20,P>0.05)、接种登记制度(H=4.25,P>0.05)、信息统计报告制度(H=0.00,P>0.05)、100%接种制度(H=1.47,P>0.05)和24 h应诊制度(H=3.20,P>0.05)得分均较高,差异无统计学意义。定性调查提示基层冷链管理制度执行较差,3个社区级门诊没有执行24 h应诊制度。接种信息共享制度未完善,致不能有效处置异地接种病例。结论成都市主城区犬伤门诊的7大业务管理制度等制度需要进一步规范和完善,有必要建立和加强接种信息共享制度,以达到成都市卫生局有关犬伤门诊的管理要求。
Objective To study the management status quo in dog-injury clinics in Chengdu, and to pro- vide references for standardization and construction of the outpatient service. Methods Both quantitative and qualitative research methods were adopted. The questionnaire survey was carried out in 21 dog-bite clinics in Chengdu, and qualitative investigation was conducted among the medical personnel. Results The 7 management systems in the clinics gained relatively high scores: cold chain management system (H= 0.75, P〉0. 05), informed consent system (H=1. 73, P〉0.05), the registration system for adverse reaction of vaccination (H=3.20, P〉0.05), registration system (H=4.25, P〉0.05), information accounting system (H =0.00, P〉0.05), 100% vaccination system (H =1.47, P〉0.05), 24 h consultation system (H= 3.20, P〉0. 05). The qualitative research revealed that the cold chain management system was poorly operated; three community clinics did not practice the 24 consultation system, and the information sharing system of vaccination was not established, which resulted in incapability of disposal on nonlocal cases. Conclusion The management systems should be further advanced and standardized in the dog-injury clinics, and thus to meet the official management requirements.
