摘要
目的:评价参麦注射液对瓣膜置换术患者血清肌钙蛋白T(TnT)及白介素-6(IL-6)、白介素-8(IL-8)的影响。方法:接受瓣膜置换术治疗的心脏瓣膜病患者38例,随机分成对照组和观察组,各19例。研究对象均接受相同的麻醉与体外循环,观察组患者于体外循环开始前静脉滴注参麦注射液。对照组患者在相同时间内静脉滴注等量的生理盐水。对麻醉后不同时间段患者的血清TnT及IL-6、IL-8水平进行对比分析。结果:T1时段,两组患者血清TnT水平比较,差异无统计学意义(P>0.05)。T2、T3、T4时段,两组患者血清TnT、IL-6、IL-8水平均显著上升,但是观察组患者血清TnT、IL-6、IL-8水平低于对照组,差异具有统计学意义(P<0.05)。结论:瓣膜置换术加用参麦注射液处理可通过控制和降低患者血清TnT及IL-6、IL-8水平,来减轻患者在治疗过程中心肌缺血-再灌注损伤和炎症反应,促进患者的预后。
Objective: To study the effect of Shenmai Injection on serum cardiac troponin T (TnT) and interleukin - 6 (IL- 6) and interleukin -8 (IL- 8) in patients with valve replacement operation. Method: 38 cases of valvular heart disease treated with valve replacement operation in our hospital in December 2010 - December 2013 were randomly divided into control group and observation group, 19 cases in each. The subjects received the same anesthesia and cardiopulmonary bypass. The observation group added intravenous drip of Shenmai Injection at the beginning of cardiopulmonary bypass. Control group used the normal saline infusion with the same amount. Comparative analysis of serum TnT after anesthesia of patients and IL -6 and IL -8 levels in different time points was carried out. Results: T1 period, the serum level of TnT in two groups had no significant difference ( P 〉 0.05). T2,T3,T4 period, serum TnT, IL -6 and IL -8 levels of patients in the two groups were significantly increased. However, serum TnT, IL - 6 and IL - 8 levels of the patients in observation group were better than those in the control group and the difference was statistically significant ( P 〈 0.05 ). Conclusion: Valve replacement operation combined with Shenmai Injection treatment can control and reduce serum TnT and IL- 6 and IL- 8 levels, reduce myocardial ischemia -reperfusion injury in patients with inflammatory reaction in the course of treatment and improve the prognosis.
