摘要
目的评价0.45T永磁MRI在膝关节慢性疼痛中的应用价值。方法对60例膝关节慢性疼痛患者计68个膝关节的CR平片、29例患者CT横断面容积扫描图像以及68个膝关节MRI扫描图像进行回顾性分析、对照;其中43例与关节镜检查进行对照分析。结果 CR检查:阳性27个膝关节,阴性41个膝关节;CT检查(29例计58个膝关节):患侧阳性23个膝关节中,其中关节积液19例、关节内及其周围囊肿7例、半月板损伤5例,关节面增生、硬化17例;阴性6个膝关节;MR检查阳性59个膝关节中,软骨损伤4例、关节积液35例、内侧半月板损伤45例、外侧半月板损伤39例、半月板半脱位5例、关节内及其周围囊肿13例;阴性9个膝关节。关节镜检查(43个关节):软骨损伤2例、关节积液23例、内和(或)外侧半月板损伤共33例、半月板半脱位5例,关节内及其周围囊肿7例等。MRI、CT、CR检查与关节镜检查结果符合率分别为83.72%(36/43)、59.09%(13/22)和11.62%(5/43)。结论低场MRI在明确膝关节慢性疼痛病因方面明显优于CR平片及CT,应列为首选检查方法;引起膝关节慢性疼痛的最主要原因为半月板损伤,其次为关节积液。
Objective To evaluate the application value of 0.45T permanent magnet MRI in identifying knee joint chronic pain. Methods Retrospective analysis and contrast were conducted in terms of CR films of 60 patients suffering from knee joint chronic pain (68 knee joints ) , cross sectional CT volume scanning images of 29 cases, and MRI scanning images of 68 knee joints. Results of 43 cases were analyzed and contrasted with those by the arthroscopy. Results CR examination :27 positive knee joints and 41 negative ones. CT examination (29 cases, 58 knee joints in all) :Of the affected 23 positive knee joints,joint effusion was detected in 19 ,joint cyst in 7 ,meniscus injury in 5 ,and hyperplasia and sclerosis of articular surface in 17. 6 were reported negative. MR examination:Of all the 59 positive knee joints, cartilage injury was found in 4 cases ,joint effusion in 35, medial meniscus injury in 45, lateral meniscus injury in 39, meniscal subluxation in 5, and joint cyst in 13.9 were found negative. Examination by arthroscopy (43 knee joints) :Cartilage injury was found in 2 cases, joint effusion in 23, meniscus ( medial and / or lateral) injury in 33 ,meniscal subluxation in 5 ,and joint cyst in 7. The coincidence rates between examinations by MRI, CT, CR and arthroscopy were respectively 83.72% { 36 / 43 ), 59.09% ( 13/22 ), and 11.62% ( 5 / 43 ). Conclusion Low field MRI is apparently superior to CR films and CT in determining pathogeny of knee joint chronic pain. It should be recommended as the first choice of examination.
