摘要
目的探讨老年食管癌患者进行不同放射治疗的临床效果。方法选择2009年1月至2013年1月保定市第二医院收治的老年食管癌患者126例,根据不同的放射治疗方式分为三组:A组(三维适形放疗)、B组(后程加速超分割放疗)、C组(常规分割放疗)各46例。对比治疗后各组患者的毒性反应、生存率、局控率、有效率。结果 A组患者的放射性肺炎发生率显著低于B组和C组(χ2=4.48,5.52,P<0.05),三组患者治疗后放射性食管炎的发生率比较差异无统计学意义(P>0.05);三组患者3年生存率、局控率比较差异有统计学意义(P<0.05),且A组患者的生存率、局控率显著高于C组(P<0.05)。结论后程加速超分割放疗和三维适形放疗的生存率、局控率较常规放疗更高,且三维适形放疗的不良反应更小、安全性更高,是较为优越的一种放疗方式。对于能耐受、体质较好的老年食管癌患者,建议采用三维适形放疗,以获得较满意的治疗效果。
Objective To investigate the clinical effects of different radiation therapies for elderly patients with esophageal cancer.Methods A total of 126 cases of elderly patients with esophageal cancer admittedin Baoding Second Hospital from Jan.2009 to Jan.2013 were divided into three groups according to the way of radiation therapy：group A （three-dimensional conformal radiotherapy）,group B （late course accelerated hyperfractionated radiotherapy）,group C （conventional fractionated radiotherapy）,with 42 cases in each group.After treatment,the toxicity reaction,survival rate,local control rate,effective rate of the three groups were compared.Results Incidence of radioactive pneumonia of group A were significantly lower than group B and group C（Х^2 =4.48,5.52,P 〈0.05）,while there were no statistically significant differences of the incidence of radiation esophagitis after treatment among the three groups （P 〉 0.05） ; the 3-year survival rates and local control rates of the three groups had statistically significant differences（P 〈 0.05）,and the 3-yearsurvival rate and local control rate of group A were significantly higher than those of group C （P 〈 0.05）.Conclusion The survival rate and local control rate of the patients treated with course accelerated hyperfractionated radiotherapy and three-dimensional conformal radiotherapy were higher than those of the patientstreated with conventional fractionated radiotherapy.And three-dimensional conformal radiotherapy is a much safer and superior way of radiotherapy with less side effects.Therefore,three-dimensional conformal radiotherapy is recommended for those elderly patients with better tolerability and physical condition,in order to obtain satisfactory therapeutic effect.
