摘要
目的 比较NP和MVP方案治疗Ⅲ、Ⅳ期NSCLC的临床疗效和毒性。方法 选择病理证实Ⅲ、Ⅳ期NSCLC 72例 ,随机分成 2组 :3 4例为NP组 ,3 8例为MVP组。NVB剂量为 2 5mg/m2 ,PDD 2 5mg/m2 ,VDS 3mg/m2 ,MMC 6mg/m2 ,3周重复 ,2周期后评价疗效 ,用上述方案进行对比治疗。结果 NP组和MVP组缓解率分别为 47 1% ( 16/3 4 )、42 1% ( 16/3 8) ,两组差异无显著性。Ⅲ、Ⅳ期缓解率分别是 5 0 %、5 2 4%和 42 8%、2 9 4%。毒副反应 :消化道反应和骨髓抑制NP组均高于MVP组。结论 NP和MVP均可做为NSCLC的一线治疗方案 ,NP方案对复治、Ⅳ期病人疗效较好 ,但骨髓抑制较重。
Objective To compare the clinical effect and toxicity of NP regimen with those of MVP regimen in the treatment of stage Ⅲ～Ⅳ NSCLC.Methods 72 patients were treated with NP regimen or MVP regimen randomization.All the patients were diagnosed pathologically or cytologically with measurable lesions.Clinical characteristics were similar and comparable.Results In 34 patients with NP regimen,the response rate(CR+PR) was 47.1%(16/34).In 38 patients with MVP regimen,the response rate(CR+PR) was 42.1%(16/38).In both groups there was no apparent difference.The toxicity in NP regimen was more severe than that in MVP.Conclusions There was definitively good therapeutic effect in NSCLC.Neutropenia was the dose-limiting toxicity in the NP regimen.But there was better effect in the patients of retreatment and stage Ⅳ.
