摘要 目的探讨食管癌术后早期肠内营养(EN)的临床应用效果。方法将42例手术治疗的食管癌患者随机分为EN组和全肠外营养(TPN)组,观察比较二者患者手术前后营养状况、手术后恢复情况及并发症发生情况。结果手术后7 d,EN组清蛋白、前清蛋白明显高于TPN组,比较有统计学差异(P均<0.05),而体质量和血红蛋白指标在两组之间比较无统计学差异(P均>0.05)。术后,与TPN组相比,EN组肛门排气时间和排便时间明显缩短,比较都有统计学意义(P均<0.05)。术后并发症方面,EN的并发症发生率为9.5%,明显低于TPN组38.1%的并发症发生率(P<0.05)。结论食管癌术后患者早期应用EN的临床效果良好,可以改善患者的营养状况,促进术后胃肠功能恢复,且并发症发生少,明显优于TPN,值得临床上进一步研究。 Objective To investigate the clinical effect of early enteral nutrition (EN) in postoperative patients with esophageal cancer. Methods 42 cases of postoperative patients with esophageal caner were randomLy divided into EN group (n=21) and total parenteral nutrition (TPN) group (n=21). Then, nutritional status, general recovery and occurrence rate of complications between the two groups were compared. Results Seven days after the operation, albumin and prealbumin in the EN group were statistically higher than those in the TPN group (both P〈0.05), while body weight and hemoglobin in the two groups were statistically same (both P〉0.05). In comparison to the TPN group, the time needed for recovery of anal exsuflfation and defecation in the EN group were statistically shorter (both P〈0.05). As to complications, the occurrence rate in the EN group was 9.5%, which was statistically lower than that of 38.1%in the TPN group (P〈0.05). Conclusion Clinical effect of early EN in postoperative patients with esophageal caner was reliable, which could improve patients’ nutritional status, promote recovery of gastrointestinal function and reduce occurrence rate of complications, with more signiifcant advantages than those of TPN.
