摘要
目的：了解真实世界过敏性紫癜患者的临床特征及中西医药物治疗临床实效,为临床诊断规范及合理用药提供参考。方法：选取全国19家三级甲等综合医院＂第一诊断＂为过敏性紫癜患者的医院信息管理系统（hospital information system,HIS）数据,使用频数分析及关联规则进行数据分析。结果：在HIS数据的2110例过敏性紫癜患者中,发病年龄多为4-12岁的儿童,男性为主;住院科室多集中在儿科、皮肤科、泌尿科;近1/3人群肾脏损害和感染,西药以抗感染药物和糖皮质激素为主,代表用药为注射用氢化可的松和注射用头孢菌素类,少数患者使用免疫抑制剂如环磷酰胺;中医药治疗以清热解毒药、活血化瘀类药物为主,代表用药为丹参注射液和雷公藤多甙;大多数患者临床转归良好。结论：过敏性紫癜的患者以儿童为主。合并症主要为肾脏损害、感染,考虑感染为主要的诱发因素;临床治疗应及时控制感染,并佐以清热解毒、活血化瘀类中药辅助治疗,若出现肾脏损害早期应及时进行治疗。
Objective： a large sample related indexes based on hospital information system（ HIS） database,to know patients with Henoch Schonlein purpura distribution,understand the application of medicine use in the real world,and to provide reference for clinical prevention and treatment. Method： Nineteen comprehensive hospital information system（ HIS） data are collected. Cleaning up the database,then analysis the patients＇ information. Using the basic description method and association rules to analysis the data. Result： By analysis the data come from HIS database we found that In2110 patients,the onset age for children of 4-12 years old,male,treatment concentrated in the pediatric department,Department of Dermatology,Department of Urology,main complications are kidney damage and infection,TCM syndrome mainly for bleeding due to blood heat syndrome,solar term in winter and spring in the solar term,drug treatment of Western medicine with anti infection drugs and sugar cortical hormone,A minority of patients using immunosuppressants.Traditional Chinese medicine with heat clearing and detoxifying,promoting blood circulation and removing blood stasis drugs. The clinical outcome of good. Conclusion： HSP mainly in children with nephritis,infection,so the clinical treatment should pay attention to control infection in time,heat clearing and detoxifying,promoting blood circulation to remove blood stasis,appear on the early renal damage and timely treatment.
