基于模糊综合评判的双相障碍早期识别研究

摘要 双相障碍(BPD)是一种严重的精神疾病,具有高复发率,高自残率和高自杀率。大多数情况下,双相情感障碍以抑郁相起病,易被误诊为单相抑郁症(UPD),从而延误治疗和预后不良。该文提出一种基于模糊综合评判的双相障碍早期识别方法。185例样本,其中77例为BPD组,108例为UPD组。筛选出22个变量作为模糊综合评判的因素集,结合因子分析方法,确定权重集及模糊关系矩阵,通过一定的模糊运算规则可得出模糊综合评判结果。敏感度为71.4%,特异性为85.2%,总正确率为79.5%。该模糊综合评价方法对于双相障碍及单相抑郁的区分效果较好,可以作为一种辅助工具来帮助早期识别双相障碍。 Bipolar disorder (BPD) is a kind of serious mental illness with high rate of recurrence, self- harm and even suicide. In most cases, BPD started with depression, which might result in its being misdiagnosed as unipolar depression (UPD) , and lead to delayed therapy treat and unfavorable prognosis. Based on fuzzy com- prehensive evaluation, a method for diagnosing BPD in early stage was proposed in this study. This study covered 185 eases, including 77 cases with BPD (BPD group) and 108 eases with UPD ( UPD group). Through the statisti- cal analysis, 22 variables were selected as the factor set of fuzzy comprehensive evaluation. The weight set and the fuzzy relationship matrix were determined by using the method of factor analysis. Then, the results of fuzzy compre- hensive evaluation can be obtained through certain fuzzy algorithm. The sensitivity of this model is 71.4% , the specificity is 85.2% , and the total accuracy is 79.5 %. The fuzzy comprehensive evaluation model can discriminate BPD and UPD and be used as an auxiliary method for detecting BPD at early stage.
