摘要
目的评估Toric硬性透气性角膜接触镜(rigid gas-permeable contact lens,RGPCL)对高度散光眼的视力矫正作用。方法收集高度散光患者33例(62眼),单纯角膜散光或眼内散光〈1.00 DC验配后环曲面RGPCL,眼内散光≥1.00 DC验配双环曲面RGPCL。对高度散光眼配戴框架镜及Toric RGPCL的视力矫正效果进行比较;对〉2.00~4.00 DC、〉4.00 DC散光眼配戴Toric RGPCL相对框架镜的矫正幅度进行比较;对伴有高度近远视、低中度近远视的散光眼配戴Toric RGPCL的视力矫正效果进行比较。结果 53眼验配后环曲面RGPCL,相对裸眼视力提高0.67±0.24,9眼验配双环曲面RGPCL,相对裸眼视力提高0.50±0.24,两种Toric RGPCL对视力的矫正作用均明显优于框架镜(t=9.21,P=0.00;t=6.49,P=0.00);〉2.00~4.00 DC、〉4.00 DC散光眼配戴Toric RGPCL后相对框架镜矫正视力分别提高0.17±0.15、0.26±0.14,两者比较差异有统计学意义(t=2.47,P=0.016);伴有不同球镜度数的高度散光眼配戴Toric RGPCL后,视力矫正效果差异无统计学意义(t=0.47,P〉0.05)。结论 Toric RGPCL对高度散光眼有理想的矫正效果,且对于〉4.00 DC散光眼矫正效果更佳,其对散光的矫正作用与球镜度无关。
Objective To evaluate the vision correction efficacy using toric rigid gas-permeable contact lens( RGPCL) in eyes with high astigmatism. Methods Thirty-three patients(62eyes) with high astigmatism were corrected with toric RGPCL. Of which,back or double surface toric RGPCL was fitted depending on the situation of astigmatism. The vision correction efficacy between toric RGPCL and frame glasses was compared. The vision correction efficacy of toric RGPCL between 2. 00- 4. 00 DC astigmatism and eyes with more than 4. 00 DC astigmatism was compared. The vision correction efficacy of toric RGPCL for astigmatism eyes accompanied with high myopia or hyperopia was also compared with low and moderate myopia or hyperopia. Results Fifty-three eyes were fitted with back surface toric RGPCL,and the vision was improved by 0.67 ± 0. 24. Nine eyes were fitted with double surface toric RGPCL,and the vision was improved by 0. 50 ± 0. 24. Both the back and double surface toric RGPCL had significant differences compared with frame glasses for high astigmatism after wearing(t = 9. 21,P = 0. 00;t = 6. 49,P = 0.00). The vision of 2. 00- 4. 00 DC astigmatism corrected by toric RGPCL was improved by 0. 17± 0. 15 relative to frame glasses while the vision of more than 4. 00 DC astigmatism was improved by 0. 26 ± 0. 14,and the difference was statistically significant(t = 2. 47,P = 0. 016). However,there was no difference in astigmatism with different spherical power(t = 0. 47,P 〉0. 05). Conclusion Toric RGPCL has ideal correction efficacy for high astigmatism,and the efficacy is better in astigmatism more than 4. 00 DC while it is irrelevant with spherical power.
