比较18F-FDG PET/CT、11C-MET PET/CT和MRI诊断神经胶质瘤的效能 被引量:5

摘要 目的比较^(18)F_FDG PET/CT、^(11)C—MET PET/CT及MRI诊断神经胶质瘤能力的差异。方法对60例临床疑似神经胶质瘤患者行^(18)F-FDG PET/CT、^(11)C-MET PET/CT和MR检查,经手术或活检获得病理诊断。以病理学结果为金标准计算^(18)F-FDG PET/CT、^(11)C—MET PET/CT及MRI诊断神经胶质瘤的敏感度、特异度和准确率,比较三者间差异。结果本组60例中,36例经病理证实为胶质瘤。^(18)F-FDG PET/CT、^(11)C—MET PET/CT和MRI的诊断灵敏度、特异度、准确率分别50.00%(18/36)、75.00%(18/24)、60.00%(36/60),97.22%(35/36)、91.67%(22/24)、95.OO%(57/60)和63.89%(23/36)、79.17%(19/24)、70.00%(42/60)。^(11)C—MET PET/CT诊断灵敏度和准确率均高于^(18)F-FDG PET/CT和MRI(P均<0.05);^(11)C—MET PET/CT诊断低级别胶质瘤(Ⅰ级和Ⅱ级)的灵敏度[94.12%(16/17)]明显高于^(18)F-FDGPET/CT[5.88%(1/17);P<0.05]。结论 ^(11)C-MET PET/CT显像诊断神经胶质瘤的能力高于MRI及^(18)F-FDG PET/CT显像。 Objective To compare the ability of 18F-FDG PET/CT, 11C-MET PET/CT and MRI for diagnosing glioma. Methods Sixty patients with suspected glioma were enrolled and underwent 18F-FDG PET/CT, 11C-MET PET/CT and MR scanning. Pathological diagnosis were obtained in all cases postoperatively or through biopsy. Taking pathology results as gold standards, the sensitivity, specificity and accuracy rate of 18 F-FDG PET/CT, 11 C-MET PET/CT and MRI in diagnosing glioma were calculated and compared. Results Among 60 cases, glioma was verified in 36 patients. The sensitivity, specificity and accuracy rate of diagnosing glioma was 50.00% (18/36), 75.00% (18/24)and 60.00% (36/60) in lSF- FDGPET/CT, 97.22% (35/36), 91.67% (22/24) and 95.00% (57/60) in 11C-MET PET/CT, 63.89% (23/36), 79.17% (19/24) and 70. 00% (42/60) in MRI, sig.nificant differences of sensitivity and accuracy rate was found between 11C-MET PET/CT and lSF-FDG PET/CT as well as MRI (all P〈0.05). The sensitivity of 11C-MET PET/CT in diagnosing low-grade (grade I and grade I/) glioma was significantly higher than that of lSF-FDG PET/CT (94.12% [16/17]vs 5.88% [1/17], P〈0.05). Conclusion Compared with MRI and 18F-FDG PET/CT, 11C-MET PET/CT demonstrated higher diagnostic ability in diagnosing glioma.
