原发性慢性肾小球疾病患者肾脏MCP-1表达的研究

摘要 目的:观察原发性慢性肾小球疾病患者肾脏单核细胞趋化蛋白-1(MCP-1)表达与肾小管间质损害、肾功能及尿蛋白分子量之间的关系,探讨MCP-1在原发性慢性肾小球疾病发病机制中的作用,为原发性慢性肾小球疾病的治疗提供部分实验依据。方法:用免疫组织化学染色法检测50例同步行肾活检诊断明确的原发性慢性肾小球疾病患者肾脏MCP-1表达,聚丙烯酰凝胶电泳(SDS-PAGE)银染法检测尿蛋白分子量。结果:原发性慢性肾小球疾病患者MCP-1表达主要位于肾小管上皮细胞内。原发性慢性肾小球疾病患者肾组织MCP-1阳性率肾小管间质病变分级3级组较1级、2级组差异有统计学意义(P<0.05),血清肌酐(Scr)>140μmol/L组较Scr≤140μmol/L组差异有统计学意义(P<0.05),混合性蛋白尿10 kd组较23 kd组差异有统计学意义(P<0.05)。结论:原发性慢性肾小球疾病患者肾组织MCP-1表达在肾小管间质病变及肾功能损害中起重要作用。10 kd等低分子量尿蛋白与肾组织MCP-1表达关系密切。可考虑使用MCP-1抗体等阻断MCP-1作用而防治原发性慢性肾小球疾病的进行性恶化。 Objective:To evaluate the role of monocyte chemoattractant protein - 1(MCP - 1)in the mechanism of progres-sion of primary chronic glomerular diseases. Methods:A total of 50 patients with biopsy proven primary chronic glomerular diseases were studied. We detect the expression of MCP - 1 in renal tissue by immunohistochemical staining method. Data were processed with the classes of tubulointerstitial lesions,serum creatinine(Scr)and molecular weight of urinary protein. Results:MCP - 1 expression was mainly in renal tubular epithelial cells. MCP - 1 expression was significantly higher in grade 3 group than in grade 1 and grade 2 group of chronic glomerular diseases patients according to classification of tubulointerstitial lesions(P ﹤ 0. 05),was significantly higher in Scr ﹥ 140 μmol/ L group than Scr≤140 μmol/ L group(P ﹤ 0. 05),was significantly higher in 10 kd proteinuria group than 23 kd proteinuria group(P ﹤ 0. 05). Conclusion:MCP - 1 may play an important role in the progression of primary chronic glomeru-lar diseases. Low molecular weight urinary protein such as 10 kd protein may have close relationship with the expression of MCP - 1 in renal tissue. The therapeutic potential of MCP - 1 targeting agents needs to be investigated in the future.
