血清内脂素水平与高血压分级相关性的初步探讨 被引量:7

Correlation between serum levels of visfatin and stages of essential hypertension

摘要 目的:探讨血清内脂素水平与高血压分级之间的相关性。方法:入选52例新诊断的高血压患者和38例健康志愿者作对照组,依据高血压分级标准将高血压组分为高血压1级和2级两个亚组,对照组分为正常血压和高血压前期两个亚组。每名入选者均测定血压、体重、身高、血脂、血糖、高敏C反应蛋白及内脂素水平。运用等级相关分析判断高血压分级与内脂素水平的相关性,进一步运用多元线性回归分析模型判断血压分级与内脂素水平之间的独立联系。结果:4个亚组间患者的年龄、血糖、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇及三酰甘油水平差异无统计学意义(P>0.05)。体质指数、收缩压、舒张压、高敏C反应蛋白及内脂素水平差异有统计学意义(P<0.05)。高血压分级与内脂素水平之间的等级相关系数r为0.819(P=0.000)。多元线性回归分析示只有收缩压水平对内脂素的影响有高度统计学意义(标准化系数0.903,P=0.000)。内脂素与收缩压的相关性有统计学意义(r=0.774,P=0.000)。结论:内脂素水平与高血压分级之间呈高度正相关。 Objective:To evaluate the correlation between serum levels of visfatin and stages of essential hyper tension(EH). Method:A total of 52 newly diagnosed hypertensive patients and 38 healthy participants were en- rolled into the study. According to blood pressure classification standard reported in the JNC 7 guidelines, hyper- tensive patients were divided into 2 subgroups: stage 1 and stage 2. The control group was divided into 2 sub- groups: normal and prehypertension. Blood pressure, weight, height, fasting lipid profile, fasting blood glucose (FBG), high sensitive C-reactive protein(hs-CRP) and visfatin levels were measured from each patient. Using the rank correlation analysis demonstrated the relationship between hypertension grade and visfatin levels, further u- sing multiple linear regression analysis model judged the independent association between blood pressure and visfa- tin levels. Result:The levels of age, FBG, total cholesterol, low density lipoprotein cholesterol, high density lipo- protein cholesterol and triglyceride were not statistically significant during the four subgroups, P^0.05. While the body mass index, systolic blood pressure(SBP), diastolic blood pressure(DBP), hs-CRP and visfatin levels difference were statistically significant, P^0.05. Rank correlation coefficient between hypertension grades and visfatin levels was 0. 819, P = 0. 000. Multiple linear regression analysis showed that only the SBP index had a highly significant effect on the visfatin(standardized coefficients was 0. 903, P= 0. 000). Visfatin levels had signif- icant partial correlation with SBP(r= 0. 774, P= 0. 000). Conclusion.. The serum levels of visfatin have a positive correlation with hypertension grades. Visfatin may be used to identify the EH, even the prehypertensive stage.
