摘要
目的探讨呼吸内科患者发生呼吸机相关性肺炎(VAP)的危险因素,制定相关对策,以减少VAP的发生率。方法选取2010年12月-2012年12月呼吸内科使用呼吸机治疗的患者46例进行分析,分别对患者年龄、性别、临床表现、机械通气时间、APACHEⅡ评分、基础疾病、住院天数和使用药物等进行分析,观察患者发生呼吸机相关肺炎的危险因素,并制定对策,所有数据采用SPSS13.0软件进行统计分析。结果 46例呼吸内科患者中有12例发生VAP,发生率26.1%;VAP的发生与患者的年龄、机械通气时间、APACHEⅡ评分、抑酸剂应用、住院天数以及患有慢性阻塞性肺疾病显著相关,差异有统计学意义(P<0.05)。结论呼吸内科患者发生VAP与多种因素密切相关,为降低呼吸机相关肺炎的发生,临床医师需要进行有效的干预。
OBJECTIVE To study the risk factors and formulate countermeasures of ventilator-associated pneumonia（VAP）in the department of respiratory medicine so as to reduce occurrence of VAP.METHODS A total of 46 patients in the department of respiratory medicine who used ventilator for treatment from Dec.2010 to Dec.2012 were selected for analysis of age,gender,clinical presentation,duration of mechanical ventilation,APACHEⅡ score,underlying diseases,length of stay and the use of drugs to observed risk factors for ventilator-associated pneumonia and the countermeasures were formulated.All data were statistically analyzed by SPSS 13.0.RESULTS Totally 12 out of 46 patients in the department of respiratory medicine had VAP,indicating the incidence was26.1%.The study indicated that patient age,prolonged ventilation ventilator,APACHEⅡ score,application of antacids,hospital stay and presence with chronic obstructive pulmonary diseases were significantly related to the occurrence of VAP,（P〈0.05）.CONCLUSION The occurrence of VAP in patients in the department of respiratory medicine is closely related to multiple factors.Clinicians should perform effective interventions to reducethe incidence of ventilator-associated pneumonia.
