摘要
目的检测20株产超广谱β-内酰胺酶(ESBLs)大肠埃希菌的β-内酰胺酶基因,以便了解该20株产ESBLs大肠埃希菌β-内酰胺酶基因亚型携带存在状况。方法收集医院2013年分离到的20株产ESBLs大肠埃希菌,采用聚合酶链反应(PCR)方法分析16种A类β-内酰胺酶基因、6种C类β-内酰胺酶基因、3种D类β-内酰胺酶基因。结果 20株产ESBLs大肠埃希菌对头孢呋辛、头孢噻肟、头孢吡肟的耐药率均为100.0%;共检出4种A类β-内酰胺酶基因,其中检出blaTEM16株、blaSHV1株、blaCTX-M-1 cluster 3株、blaCTX-M-9 cluster 14株,检出率分别为80.0%、5.0%、15.0%、70.0%,C类β-内酰胺酶基因检出blaDHA2株,检出率为10.0%,D类β-内酰胺酶基因检出blaOXA-1 cluster 3株,检出率为15.0%;20株产ESBLs大肠埃希菌每一株均有β-内酰胺酶基因检出,少者检出1种,多者同时检出2~4种,且该组菌株blaCTX-M总检出率85.0%。结论产β-内酰胺酶基因是该组大肠埃希菌对β-酰胺类抗菌药物产生耐药的重要原因。
OBJECTIVE To understand the existence condition of 20 strains ESBLs producing Escherichia coliβ-lactamase gene subtype,detectβ-lactamase gene from 20 strains extended spectrumβ-lactamase(ESBLs)producing E.coil.METHODSIsolated in 2003,20 strains of ESBLs producing E.coli in the hospital were collected,16 kinds of class A beta-lactamase genes,6kinds of class C beta-lactamase genes,and 3kinds of class D betalactamase genes were analyzed by PCR.RESULTS In 20 strains produing E.coli,the drug resistance rate was100.0%to cefuroxime,cefotaxime,cefepime,respectively.And 4kinds of class a beta-lactamase genes were positive:16strains of blaTEM(80.0%),1strain of blaSHV(5.0%),3strains of blaCTX-M-1 cluster(15.0%),14 strains of blaCTX-M-9 cluster(70.0%);1kind of class C beta-lactamase genes was positive:2strains of blaDHA(10.0%);1kind of class D beta-lactamase genes was positive:3strains of blaOXA-1 cluster(15.0%).Beta-lactamase genes were positive in every strain in 20 strains ESBLs producing E.coli:1kind of beta-lactamase gene in 4strains(20.0%),2~4kinds of beta-lactamase gene in 16strains(80.0%).Furthermore,positive rate of blaCTX-M was 85.0%.CONCLUSION Producing beta-lactamase genes in this group of E.coli played a key role in resistance to beta-lactams.
