摘要
目的：探析早期治疗贫血对改善或逆转非透析性慢性肾功能不全（CRI）患者左心室肥厚（LVH）的影响。方法：入选我院2013年1月~2014年1月门诊收治的非透析性CRI患者60例,根据就诊先后顺序不同分为观察组和对照组,各30例,观察组予以重组人促红细胞生成素（rHuEPO）皮下注射治疗,补充铁剂及叶酸,对照组仅补充等剂量铁剂及叶酸,所有患者于治疗前及治疗后检查记录舒张期左心室后壁厚度（PWT）、舒张期室间隔厚度（IVST）和舒张末期左心室内径（LVDd）,计算左心室重量指数（LVMI）,判定LVH,比较两组血红蛋白（Hb）、红细胞压积（Hct）、肾功能、血压、心率（HR）及上述各项超声心动图参数的变化。结果：治疗后观察组Hb和Hct显著升高,且显著高于对照组,差异均具有统计学意义（P〈0.05）;两组肾功能均有下降,但对照组下降更明显,差异具有统计学意义（P〈0.05）;治疗末期对照组HR显著升高,差异均具有统计学意义（P〈0.05）;治疗后观察组LVDd、IVST、PWT和LVMI均显著降低,且显著低于对照组,差异均具有统计学意义（P〈0.05）。结论：早期治疗贫血有助于改善或逆转非透析性CRI患者LVH。
Objective： To analyze the impact of early treatment for anemia on patients with left ventricular hypertrophy （LVH） of non-dialysis chronic renal insufficiency （CRI）. Method： A total of 60 patients with non-dialysis CRI admitted during January 2013 to January 2014 were chosen and divided into observation group and control group according to admission order, with 30 cases in each group. Patients in observation group were given with hepodermic injection of rHuEPO and supplement of iron and folate. Patients in control group were given simply equal dose of iron and folate. Before and after treatment, all patients were given echocardiography to record their IVST and PWT of diastolic phase and LVDd of end-diastolic phase. LVMI was counted, and LVH was judged. Hb, Hct, renal ruction, blood pressure, HR and the above changes of parameters of echocardiogram of the 2 groups were compared. Result： After treatment, Hb and Hct of observation group rose significantly while control group was declined, observation group was significantly higher than control group （all P〈0.05） ; renal function of both groups was declined, but control group was declined more significantly （P〈0. 05） ; at the end of treatment, HR of control group rose significantly （P〈0.05）; after treatment, LVDd, IVST, PWT and LVMI were all declined significantly, but control group went up, observation group was significantly lower than control group （all P〈0.05）.Conclusions.. Early treatment for anemia can help improve or reverse LVH of patients with non-dialysis CRI.
