摘要
目的使用克罗米芬(CC)微刺激促排卵方案,在体外受精与胚胎移植(IVF-ET)患者中比较CC不同使用时机对妊娠结局的影响。方法回顾性分析河南省人民医院生殖医学中心2011年1月至2013年11月期间使用了CC微刺激促排卵方案新鲜取卵患者共456周期。按CC不同使用时机分为持续使用组(184周期)、后期使用组(203周期)、早期使用5d组(69周期),比较三组患者一般情况、促排卵情况及妊娠结局的差异。结果后期使用组人绒毛膜促性腺激素(HCG)日内膜厚度、获卵数、种植率及妊娠率均显著高于持续使用组及早期使用5d组(P<0.05);三组之间HCG日雌二醇(E2)水平、黄体生成素(LH)水平及大于14mm以上卵泡数差异具统计学意义(P<0.05)。年龄、不育年限、基础卵泡刺激素(FSH)、HCG日孕酮(P)水平、未获卵周期率及可移植胚胎数均无统计学差异。结论对于使用CC的微刺激促排卵患者,后期使用CC相对能获得较好的获卵情况及妊娠结局。
Objective:To evaluate different time of using clomifene citrate on the clinical outcome of patients who underwent on mild stimulation protocol during in vitro fertilization-embryo transfer(IVF).Methods:The clinical data of a total 456 IVF cycles of infertile patients undergone from Jan.2011 to Nov.2013 in medical reproduction center of Henan Province People's Hospital were retrospectively analyzed.According to the difference time of using clomifene citrate,the cycles were divided into 3groups:group A with 184 cycles of persistent use,group B with 203 cycles of late use,group C with 69 cycles of early 5days use.The general situation of the patients,stimulating ovulation condition and clinical pregnancy outcomes in 3groups were compared.Results:The endometrial thickness on HCG injection day and number of oocytes retrieved,implantation rate and pregnancy rate were significantly higher in the group B than those in group A and group C(P〈0.05).The levels of estradiol and LH on HCG injection day,the number of follicles with diameter greater than 14 mm were significantly different(P〈0.05),but age,infertility duration,basis levels of FSH,progesterone level on HCG injection day,rate of failed obtain oocyte cycle,and number of available embryo were no significant differences among the three groups(P〉0.05).Conclusions:The late use clomifene citrate in micro-stimulation ovulation protocol could get more oocytes and better clinical pregnancy outcome.
