摘要
目的探讨血清胱抑素C(Cys C)、糖化血红蛋白(HbA1c)、24 h尿微量白蛋白(U-mAlb)在2型糖尿病肾病(DN)肾损伤诊断的临床价值。方法分析2012年9月-2013年10月在我院就诊的48例2型糖尿病肾病(DN)患者,50例2型单纯糖尿病(DM)患者及同期28例健康体检人员的血清胱抑素C、糖化血红蛋白、24h尿微量白蛋白的检测结果,并进行统计学处理。结果糖尿病肾病(DN)组的血清胱抑素C、糖化血红蛋白、24 h尿微量白蛋白检测结果明显高于其他2组,2型单纯糖尿病(DM)组检测结果高于对照组,差异有统计学意义(P<0.05);糖尿病肾病(DN)组Cys C、HbAlc及U-mAlb之间有很好的相关性(相关系数分别为0.753、0.836、0.639,P值均小于0.01)。结论2型糖尿病存在肾功能损伤时,血清胱抑素C、糖化血红蛋白和24 h尿微量白蛋白可有显著升高。以上三项联合测定,可更加灵敏地反映肾脏的损害,在糖尿病肾损伤的诊治具有重要的临床价值。
Objective To study the changes of serum cystatin C (Cys C) , glycosylated hemoglobin (HbA1 c) ,24h urina- ry albumin (U-mAlb) in type 2 diabetic nephropathy (DN) and the clinical value of renal injury. Methods A retrospective a- nalysis (September 2012 to October 2013) was conducted on 48 cases type 2 diabetic nephropathy patients(DN) ,50 cases of type 2 diabetes mellitus patients ( DM ) and 28 healthy people in terms of their detection of serum Cys C, HbA1 e, and 24h U- mAlb, and their statistics were processed. Results Serum Cys C, HbAle, and 24h U-mAlb of the DN group were significantly higher than those of the other two groups. The detection results of the DM group were higher than those of the control group. The difference was statistically significant ( P 〈 0.05 ). In the DN group, there was a good correlation between Cys C, HbAle and U-mAlb. The correlation coefficients were 0. 753,0.836, and 0. 639 respectively. Conclusion When renal injury presents in type 2 diabetes mellitus, serum Cys C, HbA1 c, and 24h U-mAlb can be significantly increased. The above three joint detection can more sensitively reflect kidney damage, and has important clinical value in diagnosis and treatment of diabetic renal injury.
