摘要
目的分析前交通动脉瘤形态学特征,揭示动脉瘤形态学特征与前交通动脉瘤破裂出血的关系。方法根据动脉瘤是否破裂将112例前交通动脉瘤患者分成破裂动脉瘤组(74例)和未破裂动脉瘤组(38例),根据三维CTA模型,记录并比较两组间的动脉瘤直径、动脉瘤容积、高宽比、尺寸比、动脉瘤角、流动角和载瘤动脉-子动脉角,采用Logistic分析动脉瘤破裂出血的独立危险因素,并利用ROC曲线判断其预测价值。结果破裂动脉瘤瘤径、高宽比、尺寸比、动脉瘤角和流动角较未破裂动脉瘤显著升高(P均<0.05),破裂动脉瘤载瘤动脉-子动脉角较未破裂动脉瘤显著降低(P<0.05),破裂动脉瘤容积与未破裂动脉瘤比较差异未见显著统计学意义(P>0.05)。Logistic分析显示高宽比、流动角和载瘤动脉-子动脉角是动脉瘤破裂出血的独立危险因素。ROC曲线分析显示,动脉瘤高宽比、流动角和载瘤动脉-子动脉角可显著预测动脉瘤破裂出血,且有中等的灵敏度和特异度。结论动脉瘤高宽比、流动角和载瘤动脉-子动脉角等动脉瘤形态学参数可协助临床评估前交通动脉瘤破裂出血的风险。
Objective To investigate the relationship between aneurysmal morphological features and rupture of anterior communicating artery aneurysms. Methods Totally 112 patients with anterior communicating artery aneurysms were divided into the ruptured aneurysrns group (74 cases) and unruptured aneurysms group (38 cases). In terms of three-dimensional CTA models, aneurysm diameter, aneurysm volume, aspect ratio, size ratio, aneurysm angle, flow angle, and parent-daughter angle were recorded and compared between the two groups. The Logistic regression analysis was used to filtrate the independent risk factor of aneurysm rupture, and the ROC curve was used to estimate the prognostic value. Results The aneurysm diameter, aspect ratio, size ratio, aneurysm angle and flow angle in the ruptured aneurysms group were much higher, while the parent-daughter angle was lower than those in the unruptured aneurysms group (all P〈 0.05). However, the aneurysm volume showed no significant difference between the two groups (P 〉 0.05). The aspect ratio, flow angle and parent-daughter angle were independent risk factors of aneurysm rupture, which markedly predicted aneurysm rupture with medium sensitivity and specificity. Conclusion Some morphological parameters such as aspect ratio, flow angle, and parent-daughter angle may help to evaluate the rupture risk with anterior communicating artery aneurysms.
