摘要
目的:调查济南军区青岛第二疗养院2010-2012年的抗菌药物使用情况,监测临床主要分离菌的耐药率,为合理使用抗菌药物提供参考。方法:对本院的抗菌药物使用情况、病原学检查及药敏试验结果进行调查并做分析。结果:2010-2012年DDDs居前24位的抗菌药物包括10种头孢菌素类、4种含β-内酰胺酶抑制剂的复方制剂、3种大环内酯类、2种头霉素类、2种碳青霉烯类、1种喹诺酮类、1种单环β-内酰胺类和1种林可霉素类。左氧氟沙星和头孢克洛的DDDs最高。在临床分离菌中最常见的5种菌为大肠杆菌、金黄色葡萄球菌、铜绿假单胞菌、肺炎克雷伯菌和鲍曼不动杆菌。除鲍曼不动杆菌以外的其他G-菌对碳青霉烯类敏感,G+菌仅对万古霉素和利奈唑胺敏感。结论:本院的细菌耐药率较高,监控和规范抗菌药物使用非常必要。
Objective: To investigate the utilization of antibacterials and moniter drug resistance of commonly isolated bac- teria in the Second Qingdao Sanitarium of Ji'nan Military Region from 2010 to 2012, so as to provide reference for rational use of antibacterials. Methods: The utilization of antibacterial agents, the results of etiological tests and drug sensitivity tests were collected and analyzed. Results: From 2010 to 2012, DDDs of the top 24 antibacterials included 10 kinds of cephalosporins, 4 kinds of compound preparations containing 13-1actamase inhibitors, 3 kinds of macrolides, 2 kinds of cephamycins, 2 kinds of car bapenems, 1 kind of quinolone, 1 kind of monocyclic lactam and 1 kind o{ lincomycin,with the DDDs of [evofloxacin and cefacIor being the highest. Escherich ia coli , Staphylococcus aureus , Pseudomonas aeruginosa , K lebsiella pneumoniae and Acinetobacter baurnannii were the most commonly isolated bacteria. Gram-negative bacteria except A. baumannii were sensitive to carbapenems,while gram-positive bacteria were sensitive to vancomycin and linezolid. Conclusion: The bacterial drug resistance rate in the hospital is relatively high. Therefore,it is absolutely necessary to supervise and standardize the utilization of antibaeterials.
