2008~2012年无菌部位肠杆菌科细菌的分离情况及耐药性分析 被引量:3

摘要 目的了解本院无菌部位标本常见肠杆菌科细菌的分布和耐药情况。方法回顾性分析2008~2012年我院临床无菌部位标本连续分离获得的常见肠杆菌科细菌对临床上常用抗菌药物的体外药敏试验结果,并用WHONET-5软件进行数据分析。结果共分离到1305株非重复肠杆菌科细菌,前5位病原菌分别为大肠埃希菌(586株,占44.9%)、肺炎克雷伯菌(413株,占31.6%)、阴沟肠杆菌(92株,占7.0%)、奇异变形杆菌(80株,占6.1%)和产气肠杆菌(34株,占2.6%)。体外药敏试验结果如下:肠杆菌科细菌对替加环素(0~9.1%)、阿米卡星(12.9%)、美洛培南(14.1%)、亚胺培南(16.8%)、厄他培南(17.5%)、阿莫西林/克拉维酸(20.5%)、哌拉西林/他唑巴坦(20.8%)、头孢哌酮/舒巴坦(21.4%)和头孢两丁(29.7%)的总耐药率较低,而其他药物基本大于40%。结论肠杆菌科细菌耐药问题严峻,临床上应加强监测其耐药变化趋势以指导合理用药。 Objective To investigate the clinical distribution and antimicrobial resistance of the Enterobacteriaceae in sterile body fluid samples. Methods Retrospective analyzed of the antimicrobial susceptibility test for Enterobacteri- aceae in sterile body fluid samples from 2008 to 2012. WHONET-5 software was used to analyze the data. Results A total of 1305 isolates were collected. The top 5 most common pathogens were Escherichia eoli (586 strains, account 44.9%), Klebsiella pneumoniae (413 strains, account 31.6%), Enterobacter cloacae (92 strains, account 2.6%), Proteus mirabilis (80 strains, account 6.1%), and Enterobaeter aerogenes (34 strains, account 2.6%). The lowest resistance rates of antibacterial agent for Enterobacteriaceae was Tigecycline (0-9.1%), and followed by Amikacin (12.9%), Meropenem (14.1%), Imipenem (16.8%), Ertapenem (17.5%), Amoxicillin/clavulanic acid (20.5%), Piperacillin/tazobactam(20.8%), Cefoperazone/Shubatan (21.4%) and Cefoxitin (29.7%), while others were over 40%. Conclusion Antimierobial resis- tance of the Enterobacteriaceae is a serious problem. We should further strengthen the surveillance of antimicrobial re- sistance for the rational use of antibacterial agent.
