摘要
目的分析我院2006—2009年肠球菌在临床标本中的分布特点及其耐药现状,为合理治疗肠球菌感染提供依据。方法常规分离菌株,采用纸片扩散法和琼脂稀释法进行药物敏感性试验,数据分别采用WHONET5.4和SPSS11.0软件进行分析和统计。结果近4年从各种临床标本中共分离到肠球菌314株,其中粪肠球菌159株,屎肠球菌131株。标本主要来源于尿液(31.5%)、痰液(28.3%)和分泌物(16.0%)。在受试抗菌药物中肠球菌对红霉素的耐药率最高(89.2%),其次为环丙沙星(65.9%),氨苄西林(59.6%)和呋喃妥因(24.2%),万古霉素和替考拉宁的耐药率最低(分别为2.0%和0.3%)。临床检出耐万古霉素的粪肠球菌,但未发现耐万古霉素的屎肠球菌。结论肠球菌在临床各类标本中的检出率有逐年上升的趋势,万古霉素、替考拉宁和利奈唑胺仍然是对肠球菌抑菌率最高的药物,但临床已检出耐万古霉素肠球菌。不同种的肠球菌其耐药性有较大差异,临床应根据药敏试验结果合理选择药物。
Objective To investigate the distribution profile and antimicrobial susceptibility ofenterococci from a tertiary teaching hospital in Chongqing from 2006 to 2009, and provide a basis for rational treatment of enterococcal infections. Methods All strains of Enterococcus were isolated from clinical specimens by routine method. The antimicrobial susceptibility was evaluated by disc diffusion test (Kirby-Bauer) and agar dilution test. Data were analyzed by WHONET5.4 and SPSS11.0 softwares. Results During the past 4 years, a total of 314 strains of Enterococci were isolated from a variety of clinical specimens, including 159 strains of Enterococcusfaecalis and 131 strains of Enterococcusfaecium. The majority of strains were isolated from urine(31.5%), sputum(28.3%), and secretion(16.0%). Among the tested antibiotial agents, erythromycin exhibited the highest resistance rate (89.2%). The resistant rates to ciprofloxacin, ampicillin and nitrofurantoin were 65.9%, 59.6% and 24.2%, respectively. Enterococci showed the lowest resistance rate to vancomycin and teicoplanin (2.0% and 0.3%). Those vancomycin- resistant Enterococci tested were Enterococcus faecalis and Enterococcus gallinarum. No Enterococcus faecium resistant to vancomycin have been found. Conclusions Vancomycin, teicoplanin and linezolid remains the most potent antibacterial agents against Enterococci. However, vancomycin-resistant enterococci have emerged. As different species of enterococci have diverse resistance profiles, rational treatment of enterococcal infections should be based on antimicrobial susceptibility test.
