降钙素原早期诊断脓毒血症的价值 被引量:13

Value of serum procalcitonin to the early diagnosis of sepsis

摘要 目的探讨血清降钙素原(procalcitonin,PCT)在脓毒血症早期诊断中的价值。方法 86例脓毒血症患者,依据病原学检测结果分为革兰阴性菌组(46例)和革兰阳性菌组(40例),依据病变程度分为严重脓毒血症组(46例)和脓毒血症组(40例),检测其血清PCT、C反应蛋白(C-reactive protein,CRP)、白细胞计数、中性粒细胞比例(neutrophil,NEU),并与50例体检健康者(对照组)进行比较。结果严重脓毒血症组PCT、CRP、白细胞计数、NEU分别为((17.34±12.32)mg/L、(34.32±13.23)mg/L、(13.98±4.45)×109/L、(88.37±21.23)%),脓毒血症组分别为((9.34±3.34)mg/L、(11.34±5.43)mg/L、(11.78±3.37)×109/L、(82.34±10.45)%),均高于对照组((0.23±0.06)mg/L、(0.68±0.23)mg/L、(7.32±2.97)×109/L、(64.43±10.45)%)(P<0.05);严重脓毒血症组PCT、CRP水平均高于脓毒血症组(P<0.05);革兰阴性菌组血清PCT、CRP水平、白细胞计数、NEU分别为((16.34±8.85)mg/L、(17.52±9.64)mg/L、(12.65±5.87)×109/L、(81.36±30.55)%)高于革兰阳性菌组((9.43±0.32)mg/L、(13.43±8.33)mg/L、(9.87±3.87)×109/L、(64.60±21.87)%)和对照组(P<0.05),革兰阳性菌组血清PCT水平高于对照组(P<0.05)。结论 PCT与脓毒血症病情严重程度及病原菌感染类型有一定关系,可用于脓毒血症的早期诊断。 Objective To study the value of serum procalcitonin (PCT) to the early diagnosis of sepsis. Methods Eighty-six patients with sepsis were divided into Gram-negative bacteria group (n=46) and Gram-positive bacteria group (n=40), and simultaneously were divided into severe sepsis group (n=46) and sepsis group (n=40) according to the disease severity. The serum PCT level, C-reactive protein level, number of white blood cells and percentage of neutrophils were detected in 86 patients, and compared with control group. Results The serum PCT level, C-reactive protein level, number of white blood cells and percentage of neutrophils were (17. 34 ±12. 32) mg/L, (34. 32 ± 13.23) mg/L, (13. 98±4.45)×10^9/L and (88.37±21.23)% in severe sepsis group, (9.34±3.34) mg/L, (11. 34± 5.43) mg/L, (11. 78 ±3. 37) ×10^9/L and (82. 34 ± 10. 45)% in sepsis group, higher than those in control group ((0.23±0.06) mg/L, (0.68±0.23) mg/L, (7.32±2.97) ×10^9/L, (64.43±10.45) %) (P〈0.05), and were higher in Gram-negative bacteria group ((16. 34 ± 8. 85) mg/L, (17. 52 ± 9. 64) rag/L, (12. 65 ± 5. 87) ×10^9/L, (81. 36 ± 30.55)%) than those in Gram-positive bacteria group ((9.43±0.32)mg/L, (13.43±8.33)mg/L, (9.87±3.87) ×10^9/L, (64.60±21.87) %) and control group (P〈0.05). The PCT level was higher in Gram-positive bacteria group than that in control group (P〈0.05). Conclusion Serum PCT level is correlated with the severity of sepsis and pathogenic bacteria type, and can be used in the early diagnosis of sepsis.
