摘要
目的通过检测2型糖尿病视网膜病变患者尿白蛋白排泄率(UAER)和血同型半胱氨酸(Hcy)水平,探讨其与发生肾损伤的关系及临床检测意义。方法选择住院糖尿病视网膜病变患者105例,按国际标准分为非增生型糖尿病视网膜病变(NPDR)25例和增生型糖尿病视网膜病变(PDR)80例,同时选择90例健康体检者为对照组,分别检测其尿素氮(BUN)、肌酐(Cr)、尿酸(UA)、空腹血糖(GLU)、Hcy和UAER水平,并进行统计分析。结果 PDR组、NPDR组的BUN、Cr、UA、Hcy、GLU、UAER与正常对照组比较差异有统计学意义(P<0.01),其中PDR组的BUN、Cr、UA、HCY、UAER显著高于NPDR组(P<0.05或0.01)。糖尿病视网膜病变组Hcy和UAER呈明显正相关关系(r=0.898,P<0.05)。结论联合检测尿白蛋白排泄率和血清同型半胱氨酸水平可作为糖尿病视网膜病变患者早期肾损伤的诊断指标。
Objective To explore the significance of detection of Homocysteine(Hcy) and urinary albumin excretion rate (UAER) in patients with diabetic retinopathy so as to observe the their relationship with renal injury.Methods 105 cases with diabetic retinopathy, including 80 cases with proliferation ( PDR group) and 25 cases with non-proliferation ( NPDR group) according to international standard.Meanwhile,90 healthy people were selected as the control group.Then, blood urea nitrogen( BUN), Creatinine( Cr), uric acid( UA), fasting blood-glucose( GLU), Hcy and UAER levels of all groups were re- spectively detected, and statistical analysis were conducted.Results The BUN, Cr, UA, GLU, Hcy and UAER levels of the PDR group and NPDR group were statistically significantly different from those of the control group ( P 〈0. 01 ) , and those of the PDR group were significantly higher than those of the NPDR group( P 〈0. 05 or 0.01).Hcy and UAER levels of PDR group and NPDR group were positively correlated ( r =0.898, P 〈0. 05).Conclusion Detection of Hey and UAER levels can be indicators in the diagnosis of early renal injury in patients with diabetic retinopathy.
